Brief Answer:
Need evaluation

Detailed Answer:

Hi,

As per your description, the child has been running fever since about 2 weeks. Fever is persistent despite 4 days of Amoxicillin. You also describe that she has nasal bleeding and that she is dull and sleeping (drowsy) most times.

Though it is not uncommon to have long duration infection, the fact that her strep infection hasn't responded adequately and presence of drowsiness + nasal bleeds are concerning. I will insist she reports back to the doctor. I would like to order a complete blood count, urine analysis along with culture studies. Perhaps a change of antibiotic is entailed. Hospitalization for supportive treatment as well as fluid administration might benefit her.

Please report back to her pediatrician for further evaluation.

Let me know if you need clarifications.

Regards
