What Could Cause Recurrent Itching Around The Anus In An Infant Despite Administering Zentel?

Question : I had asked a question on deworming my daughter aged 1 year 10 months few days back. Based on recommendation, we had given her 5ml of Zentel on Thursday( 29-Mar-2018) but again we observed that she is trying to itch near her anus multiple times.

How many days it takes after taking this medicine to work?
Also, can this medicine cause stomach upset as she is omitted couple of times today. She is otherwise normal and active. Please advise.

As mentioned earlier, We had given the medicine on Thursday night and she was normal till today afternoon. She started omitting after taking the food afternoon. Now she omitted 3 to 4 times and passed a little bit of loose motion just half an hour back.

We are trying to give her water and mostly she is omitting it back. She is otherwise normal and playing as usual.

Questions:
1. Can Zental cause omitting and loose motion episodes even after 3 days of taking.
2. I didn't understand how the Atarax drops will help here and its role. Please let me know the dosage as she is 14.5 kg.
3. Since she is omitting, what food is recommended in this condition.
3. Should we need to take her to the doctor immediately or can we wait for couple of days to see the progress.

Kindly advise.

Hello,

The effect of Zentel is usually observable in a couple of days. However, not all perianal itchiness is due to worms and unless worms are involved, a deworming drive would not really make a difference. I would insist you to wait and watch for a few more days. Further plans can be made accordingly.

Ensure that the perianal region remains clean and dry. Also, consider giving your daughter cetirizine drops (or Atarax drops) and hopefully it will help. A perianal fungal infection could also be present and if that is the case, application of Candid (clotrimazole) Dusting powder topically should help.

Regarding the vomiting episodes, Zentel can cause vomiting and stomach upset. Fluid supplementation is important. If the child is active, you need not worry.

3. Any food except for fruit juices and milk products can be given. Curd can be given. Rice based meal is preferable.

4. Wait. Monitor urine output. If urine frequency is fine, no need to worry, else increase fluid/ORS intake. If that does not suffice, you would need to take the child to a doctor.

She is now sleeping and we will monitor and see how she progress. She has omitted part of food before going to bed again.

Kindly suggest the ORS brand name, quantity and dosage to start from tomorrow if she continuous to omit along with loose stool.
