Question: My 14 year old daughter has severe abdominal pain, nausea, chest pain, shortness of breath, throat tightness, tachycardia, and constipation. She's had an abdominal and pelvic ultrasound which were normal, she was seen by a pulmonologist last week who ran two breathing test and a chest which were also normal. She's had labwork which showed elevated TSH and Testosterone levels. Any idea what could be the cause?

Brief Answer:
Possible hiatal hernia

Detailed Answer:
Hi and welcome.
this combination of respiratory and abdominal symptoms may be sign of hiatal hernia and it would not be seen on ultrasound or CT scan so it would be good to do gastroscopy. I am not sure if these hormonal imbalance is related to her symptoms but polycstic ovarian syndrome or thyroid malfunction may have some impact on abdominal pain and shortness of breath so she should do thyroid and ovarian ultrasound as well. Wish you good health. Regards

She had an abdominal and pelvic ultrasound, there were no cysts. Her
PCP said she could still have PCOS with no active cysts. We've drawn labs twice and had to ultrasounds. I have the results of all of the test I can upload, I just don't see the option to upload anything.

Her results were:
5/03/17 6/21/17
TSH: 6.14 1.90
Bilirubinm Total 1.3 N/A
T4 6.1 6.8

Brief Answer:
hi

Detailed Answer:
Hi again.
Yes, her US report seems to be normal. However, PCOS cant be excluded for sure so this is still possible disorder. But this is not very likely. Her TSH and testosterone are slightly increased but I dont think these are related to her abdominal and respiratory symptoms since these are not so high to cause any symptoms. Only very large thyroid gland may interfere with breathing and this should not be her case. It would be good to do gastroscopy as I mentioned and do food testing including gluten sensitivity...
