What Do Irregular Periods As Per My Attached Reports Indicate?

Question : Dear Doctor,
I am experiencing irregular periods since last one year,Out of which current period was after 3 months gap and it was heavy lasted for 6days.14 to 20 august.

My gynarcologist suggested sonography of whole abdomen,
I will upload detailed reports.but reports showed
1.endometrial thickness 17.6mm;Cystic endometrial hyperplasia
2.Multiple (atleast 6) large (1cm -2cm)nabothian cysts,
2.6cmfolicular cyst in right overy,
is this all serious?
I donot suffer from stomach or lower abdomen pain.Only paining takes place when there is a period.
pl suggest the necessart treatment.

I have gone carefully through your query and understand your concerns. Let me tell you that ovular cycles are painful and not anovular ones (ovum is secreted in ovular cycles, not in anovular cycles). So, the best way to reduce your pain would be to make your cycles anovular. Taking low dose hormonal pills (also known as oral contraceptive pills or OCPs) will give you the desired relief (although OCPs are otherwise used for an entirely different purpose).

In case of pain you can use Meftal P tablet (mefenamic acid) for relief. Be sure to take it in full stomach.

Dear Doctor,
I think i u have not understood my problem clearly.
Pl examine the reports attached by me .
cysts are found in my ovary.It is clearly mentioned in my report.
Pl. I want the opinion about it,whether those should be removed or not?What is the treatment to get recovered from them?
I have attached one more report of CBC which shows hemoglobin to be 9.9.
I feel tired, legs pain and fatigue.
Right now I am not experiencing any type of pain in any part of abdomen
Pl explain with respect to the reports attached.

Low dose hormonal pills should help

Detailed Answer:
Let me assure you that I have already checked the ultrasound reports and it corroborates with what I have said. The polycystic ovarian disease is responsible for the irregularity in your cycles but the painful periods are due to ovular cycles. In either case the low dose hormonal pills would provide relief. You may take mefenamic acid only if you have acute pain.

The cysts do not need to be removed and mere removal of the cyst(s) would not resolve your problems. Their treatment itself is low dose hormonal pills.

I would insist you to take iron supplements. That will help in bringing up the hemoglobin level. The low hemoglobin level is possibly responsible for your fatigue. However, you could have developed an electrolyte imbalance and that could be responsible for your muscle pain. I would insist you to take juicy fruits like citrus fruits. They contain plenty of potassium which should help in proper functioning of the muscles and in reduction of the cramps. You also need to increase total fluid (water) intake and to take plenty of ORS (oral rehydration solution) to replenish the lost fluids and salts.

Is it necessary to carry our biopsy to rule out any possibility of ovarian cancer?
How to ensure that the cyst(s) are harmless or not?
Will Keeping them in the uterus cause fuether complications?
I never expwrience any type of pain except that in period,

Thanks for writing back. Let me assure you that you need not get worried. Ovarian cysts are pretty common and do not usually indicate a cancerous cause. You need not worry.
They are not really harmless as they secrete hormones and thus mess up with the regularity of your cycles. However, if you mean harm as in cancer, let me assure you that it is not something that you need to get worried about. There is no point in getting it surgically removed unless a follow up scan shows it to be rapidly enlarging. Low dose hormonal pills should help in the gradual regression of the cyst.

The fact that you get pain only during your periods indicates that the pain is related to ovulation. Low dose hormonal pills should help.

I have already suggested the remedy. Try it out and I am certain that the condition would improve.
