Brief Answer:
This could be anything, contact dermatitis, have you been in the water/sea?

Detailed Answer:
Welcome to HCM. Could you please include what other medical conditions you may have, what medications are you taking on a regular basis. Any medications you've started recently. Or any changes in dosing. Do you have pain to this leg that has a rash. Do you have a fever. The best way to help with this answer would be to upload a picture of the rash. The you have swelling in your feet caused by heart failure? What is the condition of your toe nails? The reason I ask about your toenails is, a bacterial/fungal infection could originate from them creep up to the legs. And could confirm which leg this is happening in. Thank you and I await your response.

Brief Answer:
Yes this could be toe nail fungus creeping up.

Detailed Answer:
The treatment is prescription oral antifungals that are really needed for a long time, we are talking 1 year minimum in some cases, and then if this fails, you then see a podiatrist and if you don't care about looks and cosmetics, have the affected toe nails removed. I am assuming that you have tried the usual over the counter remedies.
