What Do Lab Test Reports Indicate?

Question: Hello Doctor

As you advised me last time I have checked my electrolyte levels which are well within range. But still today i am experiencing certain symptoms like

(1)Poor Sleep (max 5 hours)
(2)Weakness in muscles (I can do all the task and when my body is in motion i dont feel any weakness. Its only when i am sitting idle i feel as if my muscles are weak are floating and i may fall any moment)... I had never experienced such type of muscle weakness ever. I dont know what do call this phenomenon. Is it Muscle fatigue/tiredness OR its weakness i really dont know. All i can say is something is wrong because i feel my body will fall out of balance when i am sitting idle

I think i may have ADDISONS disease. I am really scared because i am experiencing similar symptoms. Please share your views on Addisons disease (reports attached)

Welcome back.

I have gone through the reports.

The reports are not suggestive of Addison's disease. In Addison's the sodium is low and potassium is high - which is not seen in your reports.

You can do an 8 am Cortisol if you still have doubts. Detailed tests for Addison's ( ACTH, stimulation tests etc) can be done only after consulting endocrinologist.

These reports are not showing any clues for your muscle pain and poor sleep. See if it improve with vitamin D supplements. If not consult a neurologist and DI detailed tests like EMG, CPK etc

Hello Doctor

I had an ABG test today as a follow up for respiratory alkolisis. As we can see in the report alkolisis has been corrected but one more thing that can be seen is the LAC value(1.38 mmo/IL) is below normal range. I would like to know whats LAC and GLU mentioned in metabolities section of the report. How will it increase and can this low LAC contribute to muscle weakness which i am experiencing now often?

Lactate and glucose

Hi,

Lac is lactate which is an acid ion in body. This will naturally be low in alkalosis - which is a compensatory mechanism in body. This is not the cause of muscle pain.

Glu is glucose - usually won't give accurate values in ABG.

Hello doctor.
i had uploaded my abg report which you didnt see. please go through it. My alkolisis has been cured so how is my lactic acid level low in the report...you said lactic acid is low in alkolisis but now i am not suffering from alkolisis. So please explain this point.

Hi,

Welcome back.

In acidosis the lactate increase and bicarbonate decrease. It is opposite in alkalosis. This is a compensatory mechanism. In mild acidosis or alkalosis the pH will be nornal, but the electrolyte change will be there. This is called compensatory acidosis and compensatory alkalosis. These changes persists for some time.

We usually say lactate as significantly low when lactate is below 0.5 mmol. Your value is 1.3 which is on the lower side but not significantly low.

Lactate levels can also be affected when the blood sample is not transported and processed properly.

So your lactate value is not a cause of muscle pain or weaknesses.

Hello doctor
Now my legs feel bettter but when i try to press the muscles and the bone then that part hurt. Although the pain is not alarming but it causes discomfort till the time the bone is pressed...could this be due to muscle fatigue/muscle tension or some other reason

Welcome back.

Muscle and bone pain need not always be due to a systemic disease. It could be a local sprain or a small injury which was un noticed.

Metabolic complications like low calcium, low potassium, high lactate is ruled out.

So have a muscle relaxant and analgesic if needed and wait for few days. Most probably it will settle by its own.
