Question: Doctor, thank you very much, im fine my bp readings frequently are 130/79,114/80,130/80,124/70,122,84,130/77.recently i have done fbs - 114, ppbs-132, FASTING lipid profile cholesterol - 218, tryglycerides - 252, ldl- 130, hdl-44
total cholesterol/hdl- 5.0. sir, i have burning sensation in my chest so i have done an ECG, which report shows, VR-105/MIN FINAL IMPRESSION - SINUS TACHYCARDIA, What does this mean, sir how to get rid of stress,

Hello again,

Sinus tachycardia is a sinus rhythm with an elevated rate of impulses, defined as a rate greater than 100 beats/min in an average adult. The normal heart rate is from 60–100 beats/min.
Increased anxiety levels are one of the most common cause of sinus tachycardia. As you heart rate is just 106, it would be termed as borderline or mild sinus tachycardia.

Your heart is made up of four chambers — two upper chambers (atria) and two lower chambers (ventricles). The rhythm of your heart is normally controlled by sino-atrial node,from there its conducted to AV node to bundle of HIS. When anything disrupts this complex system, it can cause the heart to beat too fast cause sinus tachycardia.

Triglyceride levels less then 300mg/dl does not require any medication and your cholesterol is also just marginally elevated. However Rosuvastatin 5mg can be started for a few months. Adherence to strict life style modification should be followed. Once your cholesterol and T.G levels are normal the medication can be tapered off.

For lowering your stress and controlling dyslipidemia some suggestions are --

45 minutes of strenuous exercise to be done for atleast 6 days a week for at least 3 months, by strenuous it means that you should start sweating by end of 10 minutes.

20 minutes of brisk walking at a pace not less then 5.1km/hr should be done everyday.
Avoid oily and fatty food.

Eat a lot of vegetables and high fiber food.
Increase fiber contents in your diet, take about 30-40 grams of fibers everyday.

Avoid dairy products, butter and cheese rather use olive oil for cooking.
Give up smoking and alcohol.

Drink plenty of water 2-3 liter every day.
Sleep at least 8 hours a day.

Try activities like Yoga and Aerobics.
It will take at least 3 months to observe changes.

Repeat Lipid profile after 3 months.
Keep on taking Stalopam plus as advised by your cardiologist.
If tachycardia is not relieved you can discuss with your cardiologist to add Ivabradine to your regimen which is much safer then conventionally used beta blockers in sinus tachycardia.

Both the medicines will be stopped once the things are under control by lifestyle modification.
Though not asked but burning sensation in chest can be because of gastritis.

sir i have done a ECG, Kindly pls refer attach file, also i have checked my bp a Dr. said it is 124/74, dr. i want to know in food items what to eat, fruits - what type of fruits to eat to lower cholesterol levels...... and BP LEVELS..... TO CONTROL MY HEART RATE
sir as u said i am going to walking since three months about 800metres 6-7rounds, but from one week onwards am observing in starting 3-4 round itself swetting little,
is it good.
and sir regarding TAB STALOPAM PLUS Tablet it is prescribed by a NEUROLOGIST. I HAVE taken this tab. for two days, one month ago but felt little drowsy in morning, and vomiting sensation in morng while having my breakfast. so i stopped. wat to do.....

if you are not comfortable on escitalopam plus. Ask you doctor to reduce the doses.Escitalopam plus contains clonazepam(anti anxiety) and escitalopam(SSRI).You can ask your neurologist to start you on clonazepam 0.25mg(low doses ) for 21days and if required increase the dose to 0.5mg once daily after 21days .Escitalopram should also be started initally at 5mg dose for 2 weeks and then increase the dose to 10mg if needed.
As per your query about food.
You should take 30-40grams fibres every day,oat ,wheat ,barley,jowar and ragi contains a lots of fibres.
All fresh and green vegetables are good for you all fruits can be taken but prefer fruits with high fibre contents like pear,peach,apple,oranges.
overall your diet should contain
60% carbohydrate
30% proteins
10% fats (prefer polyunsaturated and mono unsaturated fats) -olive oil,Rice XXXXXXX oil.

Your ecg is perfectly normal,heart rate is around 70/minutes,Its not tachycardia ,This ecg has been wrongly interpreted by your doctor.Your ecg is perfectly normal.

Its normal to sweat after 2-3 rounds,increase your capacity gradually and try to increase it to 2km per day.

Dr. what purpose is TAB STALOPAN PLUS 10 mg used, how does it help. after taking stalopan tab, is it posble to go to morning walk, does it gives drowziness etc., & recently i was having gastric prblm, with burning in chest, gastroenterologist prescribed SOMPRAZ 40 AND GAVISON syrup. it is decreased now. dr. can i take sompraz and gavison while taking stalopan tab. since last two months i used to eat 2 bananas daily, but when observed gastric i stopped eating bananas since two days, but now im not feeling gastric prblm . Dr. is eating banana's not good, do we get gastric problm.

Banana has nothing to do with your gastric irritation.
Yes you can have Stalopam plus with antacid and PPI prescribed by gastroentrologist.
Stalopan plus contains escitalopram (ssri) and clonazepam (anti anxiety).SSRI are usually given to lower levels of anxiety and to treat depresssion or obsessive-ness in patients,While clonazepam is an antianxiety medication and is given to relieve anxiety in patient.

Yes clonazepam makes you drowsy.Therefore it is usually taken before sleep so that the effect ends till morning.However most patient feel a bit drowsy in morning also.So going to walk depends on level of your drowsiness.

Apart from apples pears oranges .... can i take watermelon muskmelon papayya pomagranite are these friits good (Can it rrduce BP level cnstantly cholestrrl TG) is dtinking coconut water daily good how it helps.

Yes you can take all fruits and vegetables,No problem with that.These fruits contain a number of antioxidants,Vitamins and fibres which help you to stay fit and healthy.A direct relation with blood pressure and lipids is not documented in any text book,But yes in long term they are good for overall health.

I want to know the reason why your neurologist prescribed you escitalopam plus? Because making any comment on the use of this medicine is very difficult without knowing the cause why it is prescribed.
Sweating,drowsiness,malaise,fatigue etc are common side effects experienced with clonazepam and escitalopam.
If possible send me all your details that why are you on escitalopam plus.
