Question : Can someone tell me the difference, or reason why one "mass" would be overlooked, but another raise a red flag?

I've gone through your query and understand your concern,

It is difficult to interpret from limited images.
Your images may suggest tiny calcification and nodular density lesion in left breast.
CT is not ideal for breast evaluation.You may need mammography with sonography for further evaluation.
FNAC may be taken if lesion is confirmed.
Uploaded images shows normal right breast.

Other structures are normal.

If have other reports then you can upload for further discussion.
Your findings also need clinical correlation.
