Question: I took a CBC and my HBG was 12.7 last month 13.5 Hematocrit 38.3 how how bad is this?

Hi there,

This is an excellent blood count. There is no reason to be concerned. Your blood count fluctuate this much daily. 13.5 is a normal hemoglobin.

Due to your medical problems, you are at risk for anemia of chronic disease which is caused by inhibition of hemoglobin production due to inflammatory mediators from cancer. However, people with anemia of chronic disease usually have a hemoglobin less then 8. Therefore, I would not be concern about your hemoglobin. If you do begin to feel fatigued, lightheaded, short of breath, chest pain, palpations, or decrease exercise capacity, then I would have your hemoglobin rechecked.

My Platetets are 28 last month 80 last month I take Nplate shots weekly 4.4 mg It think that is the right number. usually the nplate brings them up but this time it did not. this happen before and my Hemo Dr. increased the dose of Nplate and it worked. She is out of town next week. so I will she if I can talk to who is covering for her. The hemotocrit being 38.7 Ok I know the low end is 39 . Does my age factorn

Hemoglobin was 38.3 not 38.7 I have had platelets lower then 28 before and had no problem. my WBC is over 32 ,000

Blood count is adequate for your disease process

Detailed Answer:
Hi there,

You have room to go up with the Nplate - you can go up to 10 so your doctor can adjust that appropriately. In addition, your platelets can go down to about 10 or even lower before any problems start to arise. 28 is still good for somebody with ITP and CLL cancer. I am guessing your wbc count is from CLL and it is about normal for you.

Age is not really a factor in this case. Age can be a factor, however, especially with vitamin deficient anemia's. I am guessing you have been worked up for those as well and this is more of a chronic process for you.

There is not much you can do in the meantime except wait till you see your oncologist/hematologist. Again, if you have any symptoms of anemia as I described above, or any symptoms of low platelets such as bleeding, you should be seeing earlier at the clinic.
