What Do My Genetic Test Results Indicate?

default
Posted on Sat, 31 Jan 2015
Question: Hello Doctor,

I am XXXX and my wife name is XXXX we got married in the year 2011.

1. My wife got her first pregnancy in Jan-2013 but it need to be be aborted in 45 days due to "Cardiac Activity not seen".

2. Again My wife got pregnant in Sep-2013 again we need to abort due to No Cardiac activity.

3. Then we done extensive tests like TORCH panel and Genetic tests (For me and my Wife).

4. Genetic test (Test Name: Karyotyping of Peripheral blood cells) results shows Normal condition and no problems observed though we have 4th degree ancestral relation (My Grandma from mother side) and my Wife's Grandfather (From Mothers side) are siblings.

5. But TORCH panel test taken after second Abortion shows Rubella IGG as 240 EIU\ml which Should be less than 40 EIU\ml and C.M.V IGG AB (CYTOMEGALO Virus) as 80 EIU\ml which should be Less than 40 EIU\ml.

Is Rubella IGG and C.M.V IGG AB (CYTOMEGALO Virus) are root cause of the problem? If yes how can it be prevented now and we are planning for next pregnancy? Is there any preventive vaccine for Rubella and CytoMegalo?

Also Alkaline Phosphates (SAP) observed around 550 U/L (Which should be less than 270 U/L) Can this be also problem

Thanks
XXXX

Brief Answer:
torch levels indicate old infection which is not the cause of failed preg

Detailed Answer:
Hello,
Thanks for contacting healthcaremagic.
Your first pregnancy was a missed abortion where fetal pole was seen and cardiac activity was seen but vanished later. second pregnancy was an anemryonic gestation with no fetal pole .
Also your karyotyping is normal.
The torch test has come positive but at the same time its the Rulella IgG and CMV IgG levels are raised and not IgM , so it indicates old infection which would ot cause any problem in conceiving again . There is no problem with the fertilization process. IgG levels are not of much signficance in causing a missed abortion or an anembryonic gestation unless there would have been an active TORCH infection where IgM is usually raised.

There are no particular causes which can be attributed to the previous failed pregnancy per se as the gestation was too small. It is nature's own way of selecting the best conception free from any anomalies to survival.

In my opinion you should try again and the success of pregnancy thereupon can be monitored once the fetal cardiac activity is firmly established.
Torch values are not a cause of concern as per modern obstetric practice.

Hello Doctor XXXXXXX

Thanks for quick response and we are happy with your inputs.

One more clarification.

Off-Late around 4 months my wife is experiencing right lower back in below scenario.

1. It happens when nearing the Period date and pain gradually increases when period gets delayed.

2. Pain vanishes after getting the period.

3. During pain if my wife takes some food intake then pain gets relived.

4. My wife has PCOD. Wll this be root cause?

5. Also we have done extensive scan and observed no major issues except for 2 calculi each measuring 3 mm noted in mid & lower pole calyces in Right Kidney and 3 mm calculus noted in mid pole calyx in Left Kidney. (Refer to scan report 017_Latest_Scan_Dec_14)

6. What could be the reason for this back pain? What would be remedy?

Thanks

Brief Answer:
explained....

Detailed Answer:
hello,
The pain she is experiencing few days before and ends with menses, forms a part of premenstrual syndrome complex which includes, sore tender breasts, lower back pain, mood swings, nausea, headache, even vomiting, increased frequency of urine and pain during menses ( dysmenorrhoea). This is also commonly called PMS symptms and they are common in women and can be managed conservatively with hot fomentation and and increasing water intake.
The concern about calculi is genuine ad she should increase her water intake to allow flushing out of calculi through urination itself. It can flush out if the intake is increased .It can possibly be a reason , but kidney stone pain is usually through out and not just premenstrual phase. It simply means that they are not causing any obstructive symptoms yet.

I hope i have answered yoru query in detail,
regards,
Dr. Samiir Kumar
