What Do My Lab Test Reports Indicate?

Question: I have a few thyroid nodule and a cyst on 04-2015 nodules measured, right 4.0 x 1.6 x 1.8 cm, left 3.9 x 0.8 X 1.4 cm the isthmus measures 3.5 mm then I have other nodules 2.1 x1.5 x 1.5 cm, irregular borders 2.1 x 1.5 x 1.5 cm, 0.6 x .6 x 0.3 cm a cyst 0.5 x 0. 4 x 0.2 cm.

And an ultrasound done on 11-2015 different measurements now 4.0 x 1.8 x 2.0, 3.9 x 0.3 x 1.1 cm isthmus 0.37 cm there is a slight in homogeneous nodule 2.2 x 1.3 x 1.7 cm sub centimeter cyst 5 x 4 x 3 mm and a sub centimeter 9 x 7 x 4 mm. What would be your diagnosis and the ergency of these nodules.

Hi.
Thanks for your query, a little-bit short and specific and an elucidate mentioning of the ultrasonography reports.
Both the reports show that this is multinodular goiter and needs thorough further investigations to rule out any serious disease.

Additional investigations are more important to decide the further mode of treatment and to enumerate the important ones are:
- Blood tests: Routine and Thyroid function tests.
- Ultrasonography guided FNAC of all the nodules.
These are the basic tests and further tests may be needed as per the clinical evaluation.

Since one of the nodules shown on report of 04-2015 show irregular borders and persistence of nodules on further ultrasound on 11-2015, this doesn't seem like a disease which can be controlled by any medicines.

Surgery is the answer for the most proper diagnosis as well as the treatment part.

However either way it's advisable to check into your Gen.Surgeon to get a thorough physical examination as well as clinical evaluation done.

Additional factors are:
Whether there are lymph nodes in the neck in the ultrasonography and relationship with blood vessels n color doppler studies.
Whether the goiter is seen in the neck and making it unsightly.

FNAC do you mean biopsy?
Do you mean I need surgury?
Please be honest do you think it's cancerous in reference to the lymphs?
Further testing as in?
Do you think I should schedule appointments with endochronologist ASAP?

Please do not get startled.
Let us be systematic and methodical as we are in every case.

FNAC is fine needle aspiration cytology. Here a small gauge is inserted into the nodule and aspirated to be sent to the histopathologist. If done under ultrasonography guidance the chances of getting the correct diagnosis is very high.

Need of surgery will depend upon the investigations I have mentioned.

Lymph nodes: you have not mentioned as they may not be in the report; hence I asked about whether they are present or not.

Tests as already mentioned above. Further tests depends upon the initial findings and can be thyroid scan and so on.

I think you should take the appointment with the ENT, Head and Neck Surgeon to have the most proper evaluation - not the Endocrinologist. Earlier the better.
