What Do My Lab Test Reports Indicate?

I am still in limbo with this problem of mine.
Endoscopy still shows that my sinuses do not have an infection, and my ct scans show that my sinuses are normal with no inflammation.
I still have the sinus pain in my frontal sinuses as well as a bit in my maxillary sinus and I still have trouble breathing through my nose (air passages open and close from time to time). The area where I feel pressure and pain starts from around my middle turbinate and goes up to my frontal sinus where my eyebrows sit. It slightly hurts a bit if I touch these areas.
I still have pain/tightness around my head going back to my neck.
It feels as though the majority of my sinuses are irritated rather than inflamed. Is there any condition that could possibly be causing this?
I spit out yellow/neon yellow mucus in the mornings and sometimes throughout the day. In the mornings the majority of mucus is yellow/neon yellow, and throughout the day, it will be clear, however out of the whole mucus I spit out, maybe 10-20% of it will be yellow/neon yellow.
The dr gave me a short 10 day course of prednisone (of which i only did 7 days) along with augmentin duo forte. During this time, in the mornings I would spit out green mucus.
Since the predisone, I feel as though my condition has worsened in the way that I am now feeling pressure around my ears, (was worse whilst i was taking prednisone,)however, has since died down a bit, but I hear slight crackling noises everytime I swallow (sometimes in both ears, sometimes just in the left ear)
I have had mucus cultures taken which have tested positive for MRSA. This is the only lead that I have. However the doctor has advised that it can represent asymptomatic carriage.
My tonsils are also inflamed, and have a few yellow spots on them.
I was doing daily nasal irrigation with saline bought from the pharmacy, but I feel that it inflames my sinuses rather than soothe them.
I still have the blurry vision, and i feel that when i raise my eyebrows, the tightness in my frontal sinus worsens.
If this is an MRSA infection, then I am thinking of trying Phage therapy in XXXXXXX
Can you please help me with this? I am looking for other causes that might be causing this as all the doctors tell me that it is not sinusitis. And I feel as though I am not getting the best treatment from the doctors here.
I have also attached a photo of my tonsils, please note inflammation as well as yellow specs on the tonsil. These yellow specs have been here since I originally had the tonsil infection back in September. The inflammation fluctuates, however, these yellow spots have stayed in the exact same location.
I have also attached a report of the mucus culture showing MRSA
Thanks
Upload your CT and endoscopy images.
Detailed Answer:
Hi,
Thank you for writing back.
1. I need to see your CT Scan and endoscopy images.
2. The sinus pain, pressure and congestion can be treated with supportive medication such as anti-allergic, anti-inflammatory, steam inhalation and hypertonic saline nasal washes.
3. The bright yellow colour of the mucus denotes active inflammation. The colour is derived from white blood cells.
4. You should receive a course of rifampicin along with one more of the antibiotics from the sensitivity report. The report shows that the MRSA is resistant to Augmentin. Preferably you should receive an intravenous antibiotic, in addition.
5. Do this before trying phage therapy.
6. The findings in your throat image are fairly common. Use a medicated oral rinse.
I hope that I have answered your query. If you have any more questions I will be available to answer them.
Regards.
1. I will upload my CT scan results, i will need to scan them. I will attach a copy of the report though.
As for endoscopy images, I do not have them. the doctor does not give them.
2. I do not have any congestion. that is the problem. There is slight thickening of the mucus membrane, and it is very red. That is it according to my endoscopy.
3. If that denotes inflammation, do I have reason to believe that I have an MRSA infection, even though there is no mucus coming from my nose. (Yellow mucus only when i spit it out my mouth. It is thick and yellow in the morning (I will take a photo when I can) And there are small bits of neon mucus that come out of my mouth during the day.
4. the doctor gave me a course of doxycycline, as it is still susceptible to this antibiotic. I have been advised that rifampicin is only used in a hospital setting. The doctors will not give it to me.
5. Can I safely say that my problem is a staph infection? Or could it be an allergy? I am very lost, because no ENT can tell me what is wrong with me.
6. This is NOT normal for me. I ONLY have this when I have tonsilitis. Apart from that, i never have problems.
Are the yellow specs on my tonsils bacteria? I can see the white parts as being inflammation, however, the yellow parts look like what I usually have when I have an infection. How can I get them removed?
7.Will this problem go away? This is very debilitating
8. Does the fact that the prednisone made my problem worse signify a bacterial infection?
Thanks

Detailed Answer:
Hi,
Thank you for writing back.
1. Yes, I need to see the images if possible.
2. The slight thickening of the mucous membrane and the congestion (red colour) can signify an MRSA infection. This is what I have seen in carriers who have symptoms. Typically there is a spotty inflammation (red spots, some raised) with non specific vasculitis on biopsy and a positive culture for MRSA.
3. You have MRSA as confirmed by the swabs. Mucus production is not necessary for diagnosis.
4. You need to repeat multiple swabs and if there is still MRSA, request the doctors to review.
5. From your description and test results, this is an MRSA infection. Allergy, if it co-exists, can be easily controlled with supportive medication.
6. The yellow specks on the tonsil are usually food debris trapped in the tonsillar crypts. These can also be swabbed and cultures, if there is any doubt. A medicated oral rinse will help.
7. This problem will go away with treatment.
8. Steroids including prednisolone without the correct antibiotic cover can reduce immunity and flare up an existing infection.
I hope that I have answered your query. If you have any more questions I will be available to answer them.
Regards.
1. I am unable to send through my scans, as I do not have access to a scanner at the present time.
2. I have uploaded the original report from my specialist for you to view. This was from the October last year.
3. Will treating the MRSA with antibiotics effectively treat the infection and make me better? ie lower inflammation
6. I have had enlarged tonsils with yellow debris for 6 months. This is definitely not normal for me. I have always used my tonsils as a guide to whether I am getting an infection. In the past if i felt unwell, i would look at my tonsils, and if I had inflammation or puss on them, i would take antibiotics and it would go away.
7. I certainly hope so.
8. This is why I think the prednisone made me worse. It made me worse in the sense that the inflammation/discomfort moved down the side of my face, and i now hear crackling noises in my ears when i swallow. It also brought the tightness across the top/back of my head back.
9. As I have had recurrent sinus infections over the last seven years (that would go away after a few days of applying Gentamycin nasal spray), can I assume that I have permanent biofilms in my nose that need to be eradicated? Or is each infection that I have had over the years a completely new infection?
Thanks

Detailed Answer:
Hi,
Thank you for writing back.
1. This infection, the tonsil issue will all get resolved with appropriate antibiotic therapy. Preferably this should be in a hospital setting.
2. Supportive medication as explained earlier will help control the other/ new symptoms.
3. The recent infection is a new one super-added on the previous chronic condition.
I hope that I have answered your query. If you have any more questions I will be available to answer them.
Regards.
