What Do My Lab Test Reports Indicate?

Question: My brother has large lytic cysts through out almost all of his ilian crest of the hip(which the majority are located) as well as the ball and femur. He also has bone spurs and a degenerative disc(lowest disc) He is a 35 yr old, athletic tennis teacher and has been a pro tennis player for 15 years and has played strenuous amounts of tennis. He eats and lives healthy and does not do drugs and he now has to go through 2 hip replacement surgeries and the doctor says its from wear & tear. If only I can show you this x-ray, and send you the picture, you would be shocked. There is only small amounts of bone through his entire hips(especially the ilium crest) showing large white holes that show on the mri. How can I send the mri to you? I am devastated that he has to go through this and have been relentlessly researching. Can you please tell me what direction I should look into? severe osteoarthritis? He has had tons of blood tests that all came up normal, no cancer, went to moffit cancer center and they said no etc. The cysts are almost mimicking each other but the right side is worst. Now a few months later, they found out one cyst grew 1 cm within the past several months. He has went to two orthopedic surgeons and they don't know much of what it could be except they claim )"wear & tear, and arthritis") The only things I can tell you about his past that may contribute is that he abused anabolic steroids for a few years back in 2008-2010 and would buy it from mail order through another country so I keep thinking that may have contributed because you never know what chemicals they put into that stuff. Secondly he had a bad staph infection 1 year back and it was a hard strain to treat but they did a surgery, cut a large piece of his stomach out and gave him the correct antibiotics finally. The only hip problems in our family is our grandmother had bone cancer in her hip and I had 2 slipped femoral epithysis at age of 10 (I am 31 now) which was said to be from a genetic deformity and they put a pin in the ball of each hip. If you can please allow me to send the mri picture, you would get a much better idea of the severity of this. He has continuous pain, sharp pains, pain moving to his knees and one day his leg was totally numb and couldn't use it. I desperately need your help! Thank You

Hello,
I have studied his reports and he is having bilateral hip arthritis. So hip replacement is must. But at the same time he will also need one graft to fill these cyst. So this is a little complicated surgery. He should be seen by a surgeon who is expert of revision hip surgeries. But once it is done I think he will be fine.
As far as cause of these lesions is concerned, it is very uncommon condition. It is like spontaneous osteonecrosis of acetabulum. Can be due to steroids which he took for body building. as he is not taking it now, so chances of relapse of lesion after hip surgery is very less.

Can you make an educated guess on the type of arthritis (osteo or rheumatoid)? He is 35 and we do have arthritis in the family but that is when they were 70/80 years old. Do you know why and is it common for someone his age to need two hip replacements and go through this. Also is he going to have pain for the rest of his life considering now that he has arthritis which the past orthopedic surgeon did say something about arthritis? I can't thank you enough for all your help it means a lot to me thank you so much


Also do you have an idea why one of the cyst has grown 1 centimeter with in such a short amount of time on the right side

Hello,
I think it is more like a inflammatory arthritis and it is not age dependent. Most probably it is drug induced. Once he will get hip replacement then he will not have any pain in future.Hip replacement will last for 35 to 40 years. After that he might need a revision surgery.
Growth of cyst is due to bone necrosis.So more pressure when partient walks causes large cyst in no time.
