I would kindly request you to upload these reports of the HPV test, and Pap test, and scan them using a cellphone camera. You can then upload to healthcaremagic website or you can email these reports to YYYY@YYYY Note that you might mention "Attention: Dr. Neel Kudchadkar" in the subject line of the email.
You say that your OB has recommended removal of the cervix, this can be for certain reasons that I cannot make a blanket diagnosis on, without seeing these reports.
I hope you agree that removal of the cervix is not a minor or routine medical procedure.

 Please note that also in 2015 I had the same result but my the same OB here in Dubai just totally forget to inform me. In 2013 July I had my sub total hysterectomy by laparoscopy due to Adenomyosis.
You can try laser ablation of the cervix.

If you do not wish your cervix to be removed, there are other treatments like laser therapy that destroys the unwanted tissues in the cervix, keeping the structure intact.
