Partial bowels obstruction can be the reason.
Detailed Answer:
Hello!
Thank you for the query.
From you description the problem can be in the small intestine. Such symptoms can appear due to partial bowels obstruction caused by inflammatory bowels disease. Cramps are quite characteristic for this condition. Especially if you also have temporal gas and stool obstruction (so your abdomen gets bloated before the cramps).
The second possibility is the chronic infection of intestines. Nexium (or other PPIs) when taken regularly can lead to such infection. That is why you should have stool test done at first. Avoid any milky products. And if the stool tests will be negative, CT enteroclysis and colonoscopy should be done to make sure there is no obstruction or inflammation.
Hope this helps. Feel free to ask further questions.
Regards.
