Question: 58 year male known hypertension, diabetes, EcG changes in anterior leads, TMT report says - Exercise Time-4 Min, Arrythmia- Triplet Seen, 2 d Echo report - Mild MR, AR,TR, EF= 60%, kindly suggest prognosis of the case, what will be further treatment, will it require immediate/ frequent hospitalisation

T wave inversion is there only in one lead V2,For T wave investion to be significant it should be present in atleast 2 consecutive leads.
Her 2d echo is normal.Mild MR,AR,TR is normal and is found naturally.EF-60% is excellent.
But as triplets are seen in TMT,There can be coronary artery blockage.
Most important cause of Triplets(Ventricular premature contraction in adults is coronary artery disease).

No,If patient is asymptomatic no hospitalisation is required.

Stress thallium-A thallium stress test is a nuclear imaging test that shows how well blood flows into the heart while you're exercising or at rest.During the procedure, a radioisotope (nuclear liquid) is administered through an IV.A myocardial perfusion (MIBI) scan is carried out to investigate the blood supply to the heart.It is more then 98% sensitive to rule out coronary artery disease.
TMT is about 60% sensitive to rule out CAD,combined with 2d echo sensitivity is as high as 85%.
In your case echo is normal and TMT is defective,So a thallium scan should be done to rule out possiblities of CAD.
