What Do My Lab Test Reports Indicate?

default
Posted on Mon, 23 Oct 2017
Question: Hi doctor
I have Beta Thalassemia Plus Trait (AT+) and want to marry a man with Sickle Trait (AS). Besides my blood group is O+ and my fiance' is A+. Our G6 PD is normal.
Please see the attached file of my Beta Thalassemia DNA genitics.
Please give me the possibilities of our baby if we got married.
Of course 25% our baby may have Sickle Beta thal Plus (ST +) because my DNA beta Thalassemia gene is plus as the given report shows.
Will the symptoms severe or mild? Or consider his symptoms very similar to the carrier. Will he require any blood transfusion?
What is your recommendations doctor? We love each other and want to be sure about our baby in the future if we got married.
In other words:
Is it too risky for me to marry someone with sickle cell trairt if I have beta thalassemia plus trait(AT+).
I should be grateful if u give me more details of symptoms of the coming baby if he got Sickle Beta Plus Thalassemia (ST +).According to given attached report.Which symptoms the baby may has in his life generally. Will his life like normal one. Give examples please. And our marriage will be safe or not for our coming kids.
I am looking forward to your reply
Thanks doctor.

Brief Answer:
Explained in detail.

Detailed Answer:
Hi, dear
I have gone through your question.
I can understand your concern.

First of all you have beta thalassemia trait and he has sickle cell trait.

So possibilities of your child is as following.

25% chance of normal child
25% chance of sickle trait
25% chance of beta thalassemia trait
25% chance of sickle beta thalassemia+

So in last possibility your child have symptoms.
It depends on amount of hemoglobin production. But you have beta thalassemia plus. So symptoms are not as severe as beta thalassemia zero.

Child may have frequent infection, pain attack, anemia, pulmonary symptoms etc but usually in milder form.

He may require blood transfusion but not regularly. In some illness if he develop severe anemia then only it is required.

So your child may live normal life. But in between issues regarding anemia and infection will be there.

Generally we are advised to avoid this kind of marriage. So as a medical doctor we advised to avoid this marriage.

But again if you can not live without each other then you can take risk and get marry.

Hope I have answered your question. If you have doubts then feel free to ask me. I will be happy to answer.

Dear doctor
thanks for your reply but I have some points to be explainedand clarified.
1-As there is 25% a chance of sickle beta thalassemia plus and the symptoms are mild. This means that the severity is less than 25% because this percentage including mild to severe. But mostly milder one I think as my gene is plus .
What do you think the percentage of severity in my case?
2-Besides.My grandfather has sickle beta thalassemia Plus and his life is normal like a carrier one and never requires blood transfusion.
Is the family genetical history very important. This means our gene very mild one?Is this a good signal doctor?
3-What do you mean( He may require blood transfusion but not regular)?
Do you mean he may require only in specific cases and after that case he may not require at all?

Hi, dear
I have gone through your question.

25% chance of sickle beta thalassemia plus. This percentage has no relation with severity. It is purely statistic base. As such you have plus gene that's why symptoms remains milder.

We can not say percentage of severity in any cases. Your grand father has sickle beta thalassemia plus and his life is normal. That is good sign. As such plus person have good hemoglobin formation and in general they have milder symptoms.

Your child require blood transfusion in specific cases only. This I told in comparison to normal one. Because in severe illness many times normal person doesn't require blood transfusion but sickle beta thalassemia plus person is anemic and they have oxygen carrying problem so they require blood transfusion. But it's for specific cases only. They don't require regular blood transfusion like cases of thalassemia major.

Hope I have answered your question. If you have doubts then feel free to ask me. I will be happy to answer.

Hi, dear
I have gone through your question.
I can understand your concern.

If child is sickle beta thalassemia plus and marry to normal. Then their children are either sickle cell carrier or thalassemia minor.

If he marry to sickle cell or thalassemia person then their children will be either carrier of any disease or full blown sickle cell anemia or thalassemia major.

Hope I have answered your question. If you have doubts then feel free to ask me. I will be happy to answer.

Some people try to avoid risk of getting the fetus with sickle beta thalessemia disease by doing test during the tenth week of pregnancy to be sure the fetus not got disease. If so they make abortion. Is abortion at 10th week easy and not have any side effects on mother in the future?
Thanks doctor again for your cooperation.
