What Do My Lab Test Results Indicate?

Question: Hello- I am attaching labs that show I have elevated Zinc Protoporphyrins, in addition to a normocytic, normochromic anemia. My B12 levels are elevated and elevated eosinophils. In addition, I've had low potassium, low RBC magnesium, low sodium in addition to low thyroid. I have neurologic symptoms including fine motor issues, short term memory loss, cognitive decline, aphasia and extremity weakness and was diagnosed with autoimmune encephalitis in October of 2015. My labs in the last year have shown anemia of chronic disease even when there wasn't an inflammatory state. I have extreme fatigue, night sweats, sob, wheezing, clubbing of my fingers, new onset hypertension, tachycardia, flushing of my face and profuse sweating. I am told that I don't have a the genetic form of Porphyria, but that something is wrong with my labs. I am wondering if you could look at my labs and see if they fit a Myelodysplastic picture of Hypereosinophilia syndrome or possibly even Polycythemia Vera. My dad at similar symptoms that lead to XXXXXXX extreme fatigue, heart valve damage, pulmonary HTN, CHF and end organ failure that caused his eventual death. His father had something called Osler Rendu Weber and my father was tested as a child in the 1940's and told he had this as well. Whatever is happening has been so debilitating that i have not been able to work since October of 2015 due to the cognitive issues and memory problems.

Thanks for your query. I understand your concerns.

I have gone through your entire history and the recent labs. The white cell total count and red cell counts are fine. Lymphocytes and eosinophils are relatively high which can be explained by autoimmune disorder. Extensive workup has also excluded any other malignancy.

So it is not primarily a hematologic or oncologic disorder. I believe there aren't any abnormal blood vessels anywhere in your body, which excludes Osler-Rendu-weber disease.

Autoimmune phenomena can certainly explain many of the symptoms.
