Question : Hello, I have a burning sensation and pain in lower abdomen with urination for 2 months. I was ignoring it for a while. Last week I went to my doctor and the bacterial culture was negative. I have a PDF file of my blood work and urinalysis. I wonder if I have bladder or kidney cancer? I am a 34 year old female who was otherwise relatively healthy. Protein Serum 9.9
WBC 10.2 Higher end of normal
RBC 5.22 Higher end of normal
Platelets 350 higher end of normal
Neutrophils 5.1
Lymphocytes 3.7 High
BUN 19 Higher end of normal
Creatinine 0.61 Lower end of normal
BUN/Creatinine 31 High

Urinalysis: No bacterial culture, trace of protein and 10-25 WBC in urine (found under a microscope) . They should not be there.

What should I do? What would be the next step? Do I need to see a urologist for a cytoscopy and a biopsy to rule out cancer? Or do I need to see a kidney specialist? I looked at my blood work from November and likewise numbers were off:

I would need more information on following facts-

Since when you have been suffering from Post-Traumatic Stress Syndrome?
Do you have fever with burning urination and pain in lower abdomen while urination?
How did the burning urination Start? Did it start after birth of your child?
Was the delivery normal or with any complication? Was urinary burning there before delivery of child?

Was it accompanied with Chills / rigors any time?
Was there any weight loss lately? how is your appetite?

Was there any malaise/ body ache and low grade fever in the evening, with night sweating lately in last few months time ?

Any Chest infection with Chronic Cough in the past ?as you have history of hay fever and asthma.
Any history of TB lung in your family or with you in the past ?

I would need more information as asked above,before I give any suggestions on your queries.

My Primary Impression-

Based on the facts of your query-You seem to be having Urinary Tract infection-though with Normal Urine Culture but with WBCs in Urine Analysis, which is unusual.

In this scenario-other possibilities need to be worked out,which I would suggest only after study of the above information from you.

Hello Dr. Savaskar,

I had PTSD since my father died in 2013, it was very traumatic but unrelated to my current symptoms. Maybe only general anxiety. I am doing better with my PTSD and grief.
I have no fever. If I drink a lot of water, my urination becomes not so painful.
It started a few months ago. I went to my ObGyn. My vaginal culture is normal. I am married, no STD.
My child delivery was normal.

I do get chest infections such as bronchitis 2-3x a year. I have an asthma.

I don't have TB.
How does my blood work look? Why do I have WBC (only 10-25cells) in urine culture and a trace of protein?

My BUN/Createnine ration was high for the last 2 years--range in 30s. Can it be kidney disease?

Why I have high WBC and Lymphocytes? Is it indicative of Cancer or Infection?

Which specialist do I need to see? Should I take antibiotics?

My Primary Impression and Explanation about your health issues-
Being a case of PTSD, your body is under extreme emotional trauma and stress,after death of your father.

Dear its very complex technical subject which I wanted to skip, as it needs lot of depth in the medical pahto-physiology,but I am trying to make it as simple as possible,for you to understand.
It may be very difficult to understand in the depth, in which I am explaining it to you.

I am trying to find out why have negative culture with high WBC's in your Urine Analysis.
With stress you thus go in to reduced immunity have neglected health issue for quite some time.

Your Upper and lower Urinary System is under chronic stress thereby.
High Lymphocytes indicates infection for a long period in your case.
HIGH BUN indicates stress due to PTSD and Urinary infection for a long time.

I am a stay at home mother of 4 children: 11,7,3 and 8 months. Older ones are full time in school. That keeps me very busy. I have a degree in Biochemistry and have been working full time until my third child. In 2013, I also had 2 adopted children from XXXXXXX for 2 years with severe behavioral problems such as Reactive attachment disorder. It caused severe trauma to our family, including abuse to my biological child by the adopted ones. A different family adopted them because of safety issue to the youngest child but I still have a sense of loss and failure in healing those older kids. It was around the same time as my Dad died they left our house forever. they are doing better as only children in a different family but they don't communicate with me anymore. Lots of sadness and loss. Many friends turned away from me and judge me for failing those kids. I tried to do a good thing but it almost destroyed my family. That is why I have PTSD with insomnia and nightmares too. I am trying to overcome it and not feel guilty that I have to let those children go.
I am out and about, so I am not home all the time. My kids have activities, I have many chores to do.
I am taking Albuterol for asthma as needed. 2-3x a week.
I drank coffee every day--just stopped for the last 3 days to see if it will help.
I have normal bowel movements.
My culture from urine for bacteria again came back negative. Can it be a fungal infection such as a yeast infection UTI?

On review of these updates with your blood and urine reports,please find replies to your questions which were pending still, as follows-

Regarding Replies-your Queries -

Q-1-What should I do?
Ans-You should Consult your Gyne and Get the Urine CULTURE for AFB organisms and IGRA test on Blood and Urine samples- to detect the hiedden / latent TB infection of Bladder and Urethra.
This is because -You could be suffering from Cysto-Urethritis.
This possibility is thought due to -Pyuria with Negative Urine Culture-for 2 times.
This possibility explains WBCS with negative Urine Culture.

Q-2-What would be the next step?
Ans-2-Do's Suggested-
Consult Gynec and Physician and get following plan executed under their co-operation and guidance.

-You should first get the URINE CULTURE for AFB organisms.Which would confirm TB Cysto-Urethritis- after 6 weeks when reports will come.

-In the mean while -
Take Plenty of Fluids in water /juices
Be on High Protein / bland diet
Take Antibitics-Tab Norfloxacin 400 mg Twice Daily-for 14 days after Urine Culture for AFB is sent
Tab Zicovit /Vit D3 / Vit B12 /Folic Acid to recoup general health.

This would reduce and control burning urination with lower abdominal pain mostly and would not need further care in 2 weeks time.

If no relief in complaints-
Then Cystoscopic examination with CT Contrast studies of KUB and abdomen to rule out other causes for lower abdominal pain.

Q-3-Do I need to see a urologist for a cystoscopy and a biopsy to rule out cancer? Or do I need to see a kidney specialist?

Ans-AS discussed above you would not need Cystoscopy / Urologist if things control in next 2-3 weeks time after above investigations and treatment.
But in case- you don't get any relief, you would need Urologist and Cystoscopy and even Biopsy to rule out TB cystitis,which would confirm other tests for AFB-acid-fast -bacilli.

Q-4-How does my blood work look?
Ans-Your blood work indicates chronic infection in body-that is in bladder and urine and confirms possibility of latent TB Cystitis.

I say latent-as you have not complained of any weight loss / evening rise of temperature/ loss of appetite / or AFB Sputum positivity wad detected in your case till today,as there was no suspicion due to lac of symptoms of TB lung or TB Cystitis/urethritis/Nephritis

Q-5-Why do I have WBC (only 10-25cells) in uriline culture and a trace of protein?
Ans-This unusual combination-confirms possibility of TB Cysto-Urethritis-where Pyuria accompanies negative Urine Culture and is well known reason for this combination.

Ans-Your WBC's and Lymphocytes-are high due to chronic TB Cystitis/urethritis with ?superadded infection.

Q-9- Should I take antibiotics?
Ans-You should try with increased fluids and Alkalisation of Urine- for 1-2 weeks.
If high fluids relieve your burning urination, no need of antibiotics in that case.

If burning pain is not relieved ,
Tab Pyridium 200 mg x 3 times a day x 2 weeks.
Add Tab Norfloxacin-400 mgm -1 x 2 times for 2 weeks.

If no relief still - investigate for TB Cystitis by Urine-AFB Culture and IGRA blood and urine sample testing .AFter confirmation start Anti-TB medicines- to treat it accordingly as per suggestions of Physician.

Also rule out TB Lung and TB Kidney which could be accompanied and needs to be ruled and treated if present.

How dangerous is it if I have urinary TB? Is it contagious? What is the treatment? Is it fatal?

Could it be yeast instead? Or chlamydia or gonorrhea (I am monogamous with my husband so I think it is very unlikely)?

Also, what natural herbal supplements can I take to alleviate my symptoms?
Changes in my diet?

Be patient till confirmed by AFB Urine Culture.

Hi Dear !! Thanks for the update queries
TB in itself is curable.
TB is contagious if proved to be there after all tests.So just don't be panicky till proved by tests.
Latent TB is detectable by IGRA Blood test or Urine sample tests.
If cared properly ..its not fatal and....It's Cuarble as very good Medicines are there for its cure.

-Other Causes need to be ruled out by proper and thorough investigations with your doctors.
In view of the whole facts of your query,chances of chlamydia/ gonorrhea are remote,for Sterile Pyuria in your case.
LCr test on blood and urine would fix the cause for these infections.
Possibility of False negative with Urine culture with accidental antiseptic mixed with culture broth-is to be worked up.But as you have 2 negative cultures that possibility is also remote.
Possibility of Low counts with high intake of fluids needs to be ruled out with repeat Urine Cultures.Possibility of Fungus/ yeast needs to be ruled out.
Possibility of Gonorrhea needs to be ruled out with special culture broths for Urine cultures.
Possibility of Non-infectious-causes -as Uric acid / hypercalcemia/ excess paracetamol/ Intestitial Cystitis- an autoimmune Cystourethritis with burning micturition .Sarcoidosis/ Lupus could be fixed with Cystoscopic biospy.
But all these are technical possibilities and is the function of THOROUGH INVESTIGASTIONS AND ASSESSMENT OF YOUR CASE.
So be patient and co-operate with your doctors to investigate.
Don't be Panicky and hypochondriac till then.

Suggestions-
Hence I would suggest you to discuss these possibilities with your attending Physician and Nephrologist/ Urologist, if need be.
Good high protein diet
Exposure to Sunlight daily for 20 mts
Walking exercise for 30 mts a day
Plenty of fluids and Fruit juices to improve body resistance.

Don't Confuse with herbals and complicate your health issues, which are very special and would need work up as sugggested to resolve it fast.
Tab Zincovit / Vit D3 / Vit B12 /Folic Acid and
Anti-oxidant supplements with Vit E/beta Carotene / Vit C- orally would recoup your defences.
