Question : HI I AM 37 YR OLD WOMEN AND MARRIED FOR 7 MONTHS. MY HUSBAND IS 41 YR OLD. I AM DETECTED WITH MULTIPLE FIBROIDS. LARGEST IS 105 * 67mm IN RIGHT LATERAL WALL, 80 * 61 mm IN FUNDIC REGION, 42 * 30 mm IN FUNDIC REGION ON LEFT SIDE, 38 * 34 mm AND 33 * 26 mm IN POSTERIOR WALL. a LARGE 62 * 41 mm SIZED CYST IS SEEN IN LEFT OVARY WITH THIN INTERNAL SEPTATIONS. TSH VALUE 5.95 micro IU/ ML.fREE T3 AND FREE T4 IS NORMAL. ANTI MULLERIAN HARMONE 1.4 ng/ml. PROLACTIN NORMAL. I HAVE BBEN PRESCRIBED ELTROXIN 25 mcg ONCE DAILY AND OVARES-SR ONCE DAILY (FOR 3 MONTHS). SOME DOCTORS SUGGEST OPEN OPERATION. WE WANT TO HAVE KIDS. REENTLY FOR LAST 1 MONTH I AM GETTING IRREGULAR PERIODS. eg. I HAD PERIOD FROM 7 XXXXXXX TO 13 XXXXXXX 2015 AND AGAIN ON 19 XXXXXXX 2015. SHOULD I TAKE DUPHASTON AND HOW. CAN I GET PREGNANT AND HAVE NORMAL HEALTHY BABIES WITHOUT OPERATION.

I have gone through the reports and also the history. Here are a few facts I have noticed that are significant :
....you have multiple fibroids, at least three of them are large
....you also have moderately sized ovarian cyst whose anatomy and functionality needs to be deciphered
...your hypothyroid state is perhaps related to the excess binding and this could be due to excess estrogen production; the presence of normal free hormone values suggests this
....your AMH values are in the low normal level
.....your age and your husband's age are slightly advanced and hence conception should be planned as early as possible.

Considering the fact that you have several conditions at hand that need to be dealt before you can even think of conception now, I offer a few suggestions.

1. You may continue with Thyroxine supplements and get checked every six months to see if you are taking the proper maintenance dose; else, the dose should be raised.

2. The ovarian cyst needs to be studied further. This can be done at laparoscopy.

3. If the uterus is to be conserved and to minimize the risk of uterine rupture during pregnancy and labor, laparoscopic myomectomy is the ideal procedure as compared to endoscopic or open myomectomy. This also helps to reduce recovery time.

4. Your irregular periods may be checked with hormone supplementation prescribed in proper dose by an expert. Also, maintenance of a healthy diet, healthy weight, regular exercise and avoidance of stress go a long way in regularizing the cycles.

Having offered you these suggestions, I would like to say that you have the need for an operation, i.e., laparoscopy in expert hands. Also, you may contemplate pregnancy once the ovarian cyst and fibroids are treated and also get the fertility status of your husband.
