Hello!

Welcome back on HCM!

I am glad that all your cardiac tests have resulted normal. This means that we can exclude any cardiac issues that could be possibly related to your complaints.

Coming to this point, I agree with you on the fact that costo-chondritis remains the main probable cause of your symptoms.

As Tylenol or Advil are not helpful, I would recommend other NSAIDs like Ketoprofene or Naproxene.

I would also advice taking an antiacid or PPI (omeprazole, pantoprazole, etc.), as these drugs can trigger stomach pain (heartburn and reflux). Besides, your chest pain could also be triggered by gastro-esophageal reflux or gastritis.

Some other tips to relieve the pain would be :

- gentle muscular stretching in this area 2-3 times daily
- avoid straining physical activity
- use local heat.

Blood measurements of PCR and erythrosedimentation rate are also needed to investigate for the presence of inflammation (which would be another argument in favor of chosto-chondritis).

Hope you will find this answer helpful!

If you have any other questions, please don't hesitate to ask me again!

Wishing good health,

Dr. Iliri

Hello again!

Yes, I agree that your symptoms are not related to any cardiac disorders.

The only concerning matter was high D-Dimmer levels, which could indicate possible pulmonary embolism. But, as you referred, your chest CT scan had excluded this possibility too.

Coming to this point, costochondritis or musculo-skeletal inflammation remains the most probable cause of your complaints.

I would just recommend performing inflammation tests (PCR, Erythrosedimentation rate) and CPK levels (muscular enzymes) to investigate for the above mentioned issues.

In the meantime, I would recommend trying taking Naproxen or Ketoprofen.

Hope they will help improve your situation!

Let me know about everything!

Kind regards,

Dr. Iliri

I know that the d dimer could be high for other reasons but could they be from fybromyalgia or Costco high inflammation ? Should I be worried about clotting issues

Detailed Answer:
Hello again dear!

It is true that inflammation is associated to increased D-Dimmer levels.

In this regard fibromyalgia and costochondritis, which are inflammatory disorders, can be associated to high D-Dimmer levels.

As you have performed a lung CT scan and pulmonary embolism have been excluded, there is nothing to worry about.

I would just recommend repeat this test (D-Dimmer levels) after a month, to see its progression.

It is also necessary check your rheumatoid factor plasma levels. In case of increased rheumatoid factor, D-Dimmer levels may result false positively increased.

Hope you will find this answer helpful!

Wishing all the best,

Dr. Iliri
