What Do My Torch Test Results Indicate?

Posted on Thu, 4 Jun 2015
Question: Dear doctor,

Iam a 31year old male and my wife is 28.
We had 2 early miscarriages in may 2014 and XXXXXXX 2015.

Earlier I had consulted a Urologist as we could not conceive after several attempts. I was detected with Vericocele during physical examination ~ grade 3 on left n grade 1 on right. No pain expereinced. Semen analysis showed all parameters in range. Hence the Urologist said that no further follow up is required. However I consulted a homeopath too and took some medication as a 2nd semen analysis soon after showed slighty lower motility.

After 2nd miscarriage my wife underwent following checks,
1. TORCH test. (Attached)
2. Lupus anti coagulant test.
3.ANA profile.

The TORCH test showed Rubella and CMV antibodies of past infection and the gynaec said that this MAY be the reason for 2nd miscarriage.

The gynecologist has now permitted us to try and conceive.

However after browsing through different miscarriage related write up on the internet I have stumbled on sperm DNA Fragmentation, for which vericocele is also one of the causes, and iam quite worried about it.
Earlier I had consulted 2 urologist and a homeopath and none had mentioned abt such a effect of vericocele.

1. Pls give ur views on the above issue . How can I overcome my condition
2. Wrt to my latest semen analysis report pls advice if parameters are normal.

Many thanks,
doctor

Brief Answer:
PGS may be required

Detailed Answer:
Hi,

I am sorry to hear about 2 miscarriages.

Following is my reply:

1. Miscarriages early in pregnancy (first trimester) are likely to be due to chromosomal abnormalities in the baby.

2. To prevent these possible chromosomal abnormalities you can opt for preimplantation genetic screening (PGS) of embryos after doing IVF (test tube baby) treatment. This will help to identify chromosomally normal embryos and transfer them.

3. Your semen analysis report is excellent. Hence your DNA Fragmentation is likely to be normal.

4. Your wife's CMV and other infections show past infection. This has nothing to do with miscarriage.

5. Varicocele is also not a cause for miscarriage.

6. Homeopathy may not help you much.

Let me know if you need any more help.

Regards,

Dr. Mahesh Koregol.

Hi,

You can undergo Sperm DNA fragmentation in any of best Andrology laboratories in your city.

It is difficult to say what precisely is the reason for chromosomal abnormalities. It could be hereditary or mutation. PGSwill be able to help identify the common chromosomal problems in the embryo.
