What Do Persistent Tingling Sensations In The Extremities Indicate?

Question: My 75 year old wife has a tingling to her extremities whenever she puts pressure on them Also a constant heat feeling on her right side of her entire body and is sensitive to the touch. She also has a lot of fatigue. Thank you

Good afternoon.

Your wife's symptoms are consistent with potential neurological difficulties having to do compression of nerve roots which exit the spinal cord from the cervical portion of the spinal column. This could be due to arthritic processes which can evaluated both on examination by a neurologist and confirmed by a CT or MRI of the neck.

There are other more invasive tests that could be done but on the basis of what you're writing on her symptoms I don't see the need to perform those tests unless the examining physician finds something unexpected on his exam that might prompt a more detailed analysis.

I would also consider the use of some basic laboratory tests that we always check when patients complain of symptoms having to do with tingling sensations or other problems as you're referring "heat feelings on the right side of the body, etc." In addition, fatigue could be due to metabolic issues such as a low functioning thyroid gland or anemia due to either low intake of necessary nutrients from the diet or bone marrow dysfunction which occurs more and more frequently as we age.

Other lab tests that could be helpful in making a diagnosis in your wife would be levels of Vitamin B12, Vitamin D, folate, cortisol, CBC with a differential, liver and kidney enzymes, as well as electrolytes especially Calcium and parathormone which tests the parathyroid glands that can be hyperactive in cases of extreme fatigue. I would also include a urinalysis as part of the workup since a chronic subclinical infection of the urinary tract is not uncommon and can cause symptoms of fatigue.
