Question: Hi Dr,
My mother is 55 years old and 2 years back she developed brown spots in her both legs from ankle to knee. I m attaching a pic with this. This was a sudden development which came with lot of itching and also minor watery discharge. Presently, there is no itching an any kind of discharge but the spots remain as such brown on both the legs. Can you please tell the reason for this and also prescribe a medical treatment.

I have read your query. I have seen the attached photograph also.
I understand your concern.
As per details, it seems that your mother is having a akin condition known as Scamberg' s disease or PPD (Pigmented Purpuric Dermatosis) .
It is a kind of capillaritis characterised by brown spots involving ankle area and lower legs.
In PPD, there is weaking of the vessel walls leading of oozing of blood out of capillaries which is visible on skin as brownish discolouration.
This is more common in people with long standing job, hanging legs for long hours.
In those who have varicose veins also have more chances of getting PPD.
In such cases, I prefer to advise topical steroid like hydrocortisone over the affected area.
Avoid long standing and long hours of hangigng of legs.
She should put pillow below legs while sleeping .
Regular exercise of raising legs is advised.
Tablets like vitamin C and pentoxyphylline help in strengthening of vessel wall and thinning of blood.
You should visit your doctor so that he can do clinical examination of the area and give you prescription.
You can share my opinion with him. I hope it helps.

Hello Dr.

Thanks for the response. Is topical steroid like hydrocortisone an ointment to be applied or are these tablets? Is this the name of the medicine or the salt ? Please suggest dosage (no. of times in a day and for how long)? Is it a serious condition?

Hydrocortisone is the salt present in the cream I prefer to advise to my such patients.
It is advised to be applied over the affected area two times a day in a very small amount.
These medications and creams should be used only under supervision of a doctor.

It is not a serious condition. It is more of a cosmetic concern.
