Brief Answer:
Please upload your resting ECG images, by attachment to this question.

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

Could you please upload a photo of your resting ECG? You can do this easily by attaching it as report on this question. (It is not allowed to use email addresses on this site). I would be happy to review it for a second professional opinion!

What are your symptoms?

Why did you perform a resting ECG, or was it just a routine check up by your family doctor?

Do you have any troubles like chest pain, shortness of breath, palpitations, etc.?

I remain at your disposal for further questions and to give my opinion on your uploaded tests.

Kind regards,

Dr. Iliri

I would explain as follows:

Detailed Answer:
Hello again,

I carefully reviewed your uploaded ECG and would like to explain that the main rhythm pattern is junctional rhythm (presence of inverted P wave with PR interval <120ms) with the presence of a single extra-systole.

This kind of heart rhythm may be seen in normal adults especially in young individuals, when there is an increased vagal tone, but may also be a sign of abnormalities inside the specific cardiac conductance system (sick sinus syndrome).

Also, certain drugs may lead to such an ECG pattern.

Coming to this point, I would recommend that the best strategy to follow, would be discussing with your doctor on the possibility of checking some further medical tests as follows:

- Cardiac ultrasound (ECHO)
- Ambulatory 24 to 48 hour ECG monitoring (Holter),
- Thyroid function tests,
- Pulmonary function tests (including also arterial blood gas analysis).

If no any significant abnormality results from the above tests, then you shouldn’t worry too much about (as it may be just an increased vagal tone that suppresses sinus node command and triggers lower focus generation of cardiac impulses).

In such case, I would recommend performing an exercise cardiac stress test to confirm the reversion of cardiac rhythm to normal sinus during physical strain (which is coupled with increased sympathetic tone and decreased parasympathetic nervous system drive).

Now coming to the issue of using Phentermine for weight loss, I personally would not encourage that, as this drug has already know serious cardiac adverse effects like primary pulmonary hypertension, tachycardia, palpitation, heart valve disorders (may lead to their permanent impairment), hypertension, etc.

Instead, I would encourage following a healthy life-style: a lot of every day physical activity (at least 4 km/day), a healthy diet (Mediterranean diet is strongly recommended), avoiding smoking and frequent heavy alcoholic drinks).

At the end, I would explain that ECG is a valuable tool for detecting rhythm disorders, but it should always coupled with additional tests in order to arrive to the right diagnosis.

Hope you will find this answer helpful!

In case of any further uncertainties, feel free to ask me again at any time.

Kind regards,

Dr. Iliri

Brief Answer:
My opinion as follows:

Detailed Answer:
Hi,

As you don't have any obvious complaints at all; no symptomatology, then it doesn't seem any health disorder to be present.

I would not use the term imminent, but it is recommended to perform those tests, as in such way would be possible to prevent any secondary reason for those ECG changes and prevent certain future health disorders.

I advise you to discuss with your doctor on the above scheduled diagnostic strategy and program to proceed with those tests at the most suitalbe time for you without haste.

Regards,

Dr. Iliri
