What Do The Following HIV Test Results Indicate?

Question: Dear Sir / Madam

My mother got HIV through my father. We came to know on April, 2011. She is 59 years old now.

From 2011 she is regular getting required test for CD4 count and she is having good CD4 count till date, so doctors told she is elite controller, not prescribed any medicine. only one flu injection is being given every year. She is not having any other health issues as on date.

CD4 Count record as below on these years.
20/07/2011 - 1183
08/09/2012 - 980
18/03/2013 - 638
25/09/2013 - 872
07/08/2-14 - 868
02/12/2014 - 829
05/12/2015 - 797
24/04/2016 - 852

Kindly advice us what time / at what CD4 count HIV treatment is to be started ?
What is the best treatment is available in XXXXXXX as on date, if we are ready to afford expenditure ?

Anti-retroviral treatment for HIV infection started if CD4 count falls below 350 and all the drugs are of high quality so don't get worried or tensed about it but you don't need treatment if CD4 count remains good above 500 as in case with your mother also.

The CD4 count that you have mentioned are above 500 every time so what you need to do is just take care of her nutrition and consult your physician if any symptoms will develop. Do regular CD4 count every 6 months. For any query get back to me any time. Be relaxed and move ahead in life happily.

But some good doctors / Articles says as below about treatment

" Antiretroviral therapy (ART) is the use of HIV medicines to treat HIV infection. ART is recommended for everyone infected with HIV. ART helps people with HIV live longer, healthier lives"

"HIV treatment guidelines, both in the UK and elsewhere in the world, now recommend that all people with HIV should take HIV treatment, regardless of their CD4 count.If your CD4 cell count is below 200, it is especially important that you begin HIV treatment as soon as possible. The lower your CD4 cell count, the greater the risk of HIV making you ill. But even at higher CD4 cell counts, above 500, treatment you take now will protect your health, both now and in the future"

My mother CD4 count is being maintained good from last five years without any medicine.

We are in dilemma about either treatment is good or bad at this stage. So kindly clarify my doubt in detail.

You are absolutely right that use of ART in infected patient will improve the quality of life as well as it also imlroves survival but when to start treatment depends totally on CD4 count and stage of HIV.

If you are in stage 3 & 4 then we have to start ART immidiately irrespective of CD4 count but for stage 1 & 2 we start ART only when CD4 count fall below 350 so don't get confused about the treatment.

Calm down and take care about the nutrition of the mother by giving nutritious high protein diet as well as protein supplements. For any further query you can ask me any time and I am here to help you.

Kindly brief me definition of stage 1,2,3 and 4. How can categorise any HIV person into a particular stage ?

As per your understanding my mother will come into which category ?

Please brief me for my understanding and future course of action

The definition of HIV stages depends on symptom and other opportunistic infections like tuberculosis and your mother is in first stage. At present no any treatment is required. Do follow-up CD4 at regular interval and consult physician if any symptom will develop. Get back to me again if any query left.
