What Do The Following Sonography Reports Indicate?

Posted on Wed, 14 Dec 2016
Question: Hello docter, this is in reference to my wife's screening test who as per LMP is 17 weeks and 5 days pregrant, her down syndrome results are 1:64, age risk is 1:892, the cut off being 1:250. Her bio chemical TTT marker test results , ie concentration(unit) and Corr.Mom for second trisemister are as follows
Hafp:34 U/ml (0.87)
Free hCGbeta: 70.4ng/mL(4.58)
uE3: 3.76 nmol/L(0.88)
Her sonography results are perfectly fine, her age is 30, weight 49 kgs and she is 5'3
My question is based on Free hCGbeta concentration(unit) reading ie 70.4 ng/mL and median being 4.58. whether the median calculation is correct since she is in her early second trimester and the Free hCGbeta i believe drops drastically in the second trimester. The median considered in her case is i believe for a lady who is in her 19th or 20th week and hence the variation.
I would request your feedback for the same. We are obviously doing a retest.

Brief Answer:
The levels are still high- more than normal.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
The free hCG levels drop to negligible levels ( less than 25 ng/ml) after 14 weeks. Even if the levels are tested 2 weeks earlier, the change in reference levels will not be significant. In your wife's cases the free beta hCG levels are high.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.

Dear doctor,
Thank you very much for your reply, could you please let me know as to what could be the possible interpretation considering the beta hcg values are high and the other two ie uE3 and HAfp seems okay. In the triple marker test there was also a comment who indicated nueral tube defects as low

Brief Answer:
Chances of Down's syndrome are present.

Detailed Answer:
Hello.
Thanks for writing again.
Since the free beta HCG levels are high, the chances of Down"s syndrome are there. The predictability depends on the three factors taken together. A predictability of 1:64 is high.
Hope my answer is helpful.
