Question : dear Doctor ,
kindly guide as per given discharge summery and investigation reports -
1) is treatment diagnostic hysterectomy with 3 d laproscopic myomectomy with intercede placement is related to treatment for infertility.
2) as per USG report incidental finding of multiple fibroid and partially septate uterus .
3) in month of July 2016 patient was 6.2 week primi gravida and underwent suction evacuation followed by check hysterectomy .
4) in month of September 2016 patient was admitted for diagnostic hysteroscopy with 3 D laparoscopic Myomectomy with intercede placement .

kindly guide weather the admission in month of September 2016 is for infertility treatment.
