Brief Answer:
LVAD followed by cardiac transplant.

Detailed Answer:
Hi
A per your query yes drugs are ok,
But as EF is not improving increase the doses cardivas from 3.125mg BD to 6.25mg BD this week till we reach an optimal dose of 12.5mg twice daily.
Ejection fraction wil be raise only when patient is given optimal doses.

Secondly ask your doctor to add trimetazidine 35mg twice daily for few months. It will help to increase the EF.
Apart from that all other drugs are ok.
If post CRTD also EF is not picking up then Left ventricular assisting device followed by a cardiac transplant is a good option.EF of around 15% makes patient prone to sudden cardiac death anytime.

Thanks.
