Brief Answer:
Radical orchidectomy
Detailed Answer:
Dear Sir,
Your nephew seems to be having testicular tumor with raised biochemical markers. You don't have a choice. He certainly needs to be subjected to surgery and then follow up with markers as well as CT scan. He may have to receive chemotherapy or radiotherapy also depending on the report of removed testis and the above mentioned investigations after 3 months.
Regarding fertility most of the people are fertile even after one testis is removed. However, if you are highly concerned about the fertility then you can preserve his semen now and it can be used at later date if any issues arise.
Currently your primary aim should be saving the life rather than fertility and hence surgery is must. Testicular tumor if treated appropriately and in time has almost 100% cure.
Take care
Sincerely
