What Do The Following Ultrasound Reports Of Breasts And Pelvis Indicate?

Question: I wanted an opinion , if possible on results of my breast ultrasound and my pelvic ultrasound, it reads as follow: In the right breast at 12 o'clock, there is a small 3mm cyst. At 2 o' clock left breast, there is a 4x3 mm cyst. There is a cluster of small cysts seen at 3 o' clock in the left retroareolar region. BI- RADS 2 : Benign finding. Recommendation Routine follow up in 1 year is recommended.

Female Pelvic Ultrasound:
IMPRESSION:
1. Heterogeneous Uterus
2. Four Uterine fibroids, the largest of which measures 2.3CM in greastest Diameter
3. 1.5 CM Complex cyst or Right ovary
4. 1.7 CM cyst of left ovary.

Recommendation : Assess Clinically

I was concerned about the complex cyst. And I want to know if the fibroids are the cause of causing me to have an urge to urinate often. Thanks for your time.

Thanks for your query and posting the reports.

Read the reports and your information about the problem of urge to urinate often.

1. Ultrasonography of the breasts BI-RADS 2 means that finding was normal (That is there is no cancer), but other findings (eg, cysts) are described in the report. You should continue your routine screening. This is most probably a fibrocystic disease of the breast , completely benign and never turns into a cancer.

2. Female Pelvic Ultrasound ::
Yes, complex cyst has to be invested further.
I would recommend in such a case MRI or CT scan. Since the age is 37 I would recommend to have diagnostic laparoscopy and excision of the cyst for histo-pathological examination.

Fibroids do not cause symptoms of urge to urinate in normal circumstances.

Hi doctor thanks for your quick reply. So you recommend both the MRI and the laparoscopy or MRI (or scan) first? I do have to mention that I do have heavy bleeding and cramps. I have been having an urge to urinate for 3-4 weeks now. Went to ER 1/2 week ago and was diagnosed with UTI and yeast infection. I took antibiotics as prescribed but now I find I have to get up 1-2 a night to urinate and that never happened..

Your feedback makes the things more clear.
I would recommend the MRI first.
Wait and see the results of the MRI.
Since you have also a history of heavy bleeding and cramps, which most probably are due to the fibroids, further decision should be taken in association of the Gynecologist.
Gynecologist would examine you in detail and also see for the possibility of a Cystocele which can be an added reason for sustained urge to urinate in spite of the antibiotic cover you have received for UTI.

The final decision will now depend upon the report of the MRI, clinical findings by the Gynecologist on actual examination.

Laparoscopic procedure will be decided at the end, and whether to go for Hysterectomy and removal of the testis with complex cyst of the right ovary, in consultation with your Gynecologist.

Thanks again. One last question o basically an explanation to see if we can get to the root of this. Last March, I had my gallbladder removed, the surgery went well, but i had a complication where I wasn't able to urinate, so 3 days after surgery I was very sick and had to be admitted and a catheter was placed. 3 days later when they removed the catheter I was unable to urinate, 4 weeks passed by with taking out catheter and putting it back in because I couldn't urinate, and then at 4th week when I was finally able to, i went back to ER with a bad UTI. I was given strong antibiotics twice which cause for my to develop yeast infections which are coming and going since last May. Since I was having pelvic pain the doctor had ordered the ultrasound where the fibroids where discovered but nothing read "complex" until this 2nd ultrasound. This time they also did an abdominal ultrasound which showed an 0.6cm non obstructing calculus stone on my left kidney (don't know if this would also have anything to do with all of this) Thanks again for your time!

Thanks for your detailed history and feedback.
Certainly the Small kidney stone now works as a nidus of recurrent infection.
The history of inability to pass urine after the gall bladder surgery, catheterization, and further history of antibiotics and yeast infection has to be well investigated by the Urologist as such complications are almost unheard of in a patient of 37 undergoing gall bladder surgery.

MRI will help to differentiate and clarify the complex cyst well.
Examination and examination by a combined interacting Team of Urologist and Gynecologist will help you to get a proper diagnosis and plan for further treatment so that you are cured of the problems.
