Question: Hello Doctors. I had a high risk encounter with a sex worker in Panama bout 3 month ago. im not sure what we did I was very drunk and don't remember exactly what we did if anything i might have gone down on her but I'm not sure. I got an STD panel done that had HEP B and C HIV Antibody blood test RPR for Syphilis HSV1 HSV 2 Chlamydiae and Gonorrhea all were non detectied or non reactive accept the hep C which had a 0.02 titter. I got tested again at two months for HEP B and C which had a titter of 0.01 Had a 4th gen. antigen antibody 1/2 HIV test non reactive and RPR for Syphilis with confirmatory non reactive . Over this time period i have been very stressed iv lost almost 30 pounds in the last 3 months, I developed trush at about 4 weeks after the encounter and around the same time had a small dry dark mole on my penis which has faded, and in the past couple of days 2 pin head dark spots on the head of my penis. i haven't noticed any cancer sore on my penis or in or around my mouth. I haven't felt sick accept for two days after the encounter on the boat i work on it was rough seas and i had nite sweats that nite for a couple of hours and got pain in the left side of my pelvis that lasted a few days, I get motion sickness bad. I haven't had and fevers or diarrhea or vomiting or Nausea. I have psoriasis and it cleared up on my hand about a month after the encounter . 1 can I trust my test results at 8 weeks? 2, can all the symptoms I've had be from stress? My Stress level has been to the point of Suicidal. Any thing will help. Thanks so much

Brief Answer:
Syphilis ruled out. Repeat HIV, Hepatitis test after 12 weeks.

Detailed Answer:
Hi,
Welcome to HCM.

Thanks for posting your query.
First of all, not every sex worker is having STD/HIV to infect others. They would also knew their risk and would take adequate precautions.
Your non reactive RPR result is final by 8 weeks and you are free from Syphilis. Hepatitis B and C can be detected within 8 weeks by the investigations, may be delayed in few.
Regarding HIV the test would be more conclusive by 12 weeks.
Non of your penile symptom has any relation to HIV. Dark spots on penis could be due to early HPV infection. Only an examination of your penis can either confirm or rule out the same.
There is nothing to get anxious about STD/HIV. Treatment is available for all diseases. Get out of your suicidal tendency. These are also like any other diseases.
Always try to stick to safe sex in future and avoid alcohol up to intoxication to remain free from unnecessary mental tension. Repeat the tests for viral STDs (HIV,HEPATITIS B ad C) at 12 weeks after your risky contact and rule out these possibilities once again and relax yourself. Have a consultation with a Dermatovenereologist for the dark spots over penile head.

what about the trush what could have caused that to come up? its a mild case and i am a smoker and i do drink as you already know.
does it look good for a negative on the hep c because the count went down from 0.02 to 0.01 and what could have caused it to show up at all. What scenarios are there to cause a false negative in the HIV 4th gen, and the RPR. thank you so much Dr XXXXXXX

Brief Answer:
Tongue coating can occur with smokers, not oral thrush.

Detailed Answer:
Hi,
Welcome back.
Coating of tongue is different from Oral thrush. Coating can occur in various conditions like smoking, fever, toxic conditions etc. whereas oral thrush is caused by Yeast fungus (Candida Albicans) when the immunity of the individual go down or when there is any other local disease of mouth over which the growth of this fungus get encouraged. It can occur in conditions other than HIV also. It is not difficult for a physician to differentiate coating from oral thrush. You consult your physician for further help.

Brief Answer:
Stress is not a cause for oral thrush.Bloody stools is not specific for HIV

Detailed Answer:
Hi,
Welcome back.
Oral thrush is caused by yeast (Candiada Albicans - a fungal infection) infection when there is immune deficiency or other predisposing factors. It would not occur due to stress. But because of stress and anxiety something else would likely be to look like Oral thrush for you.

Blood stain stools can occur to anybody even without HIV due to any intestinal infection. It would be treated easily after some investigations on stools.
