Question : Hello Doctors. I had a high risk encounter with a sex worker in Panama bout 3 month ago. im not sure what we did I was very drunk and don't remember exactly what we did if anything i might have gone down on her but I'm not sure. I got an STD panel done that had HEP B and C HIV Antibody blood test RPR for Syphilis HSV1 HSV 2 Chlamydiae and Gonorrhea all were non detectied or non reactive accept the hep C which had a 0.02 titter. I got tested again at two months for HEP B and C which had a titter of 0.01 Had a 4th gen. antigen antibody 1/2 HIV test non reactive and RPR for Syphilis with confirmatory non reactive . Over this time period i have been very stressed iv lost almost 30 pounds in the last 3 months, I developed trush at about 4 weeks after the encounter and around the same time had a small dry dark mole on my penis which has faded, and in the past couple of days 2 pin head dark spots on the head of my penis. i haven't noticed any cancer sore on my penis or in or around my mouth. I haven't felt sick accept for two days after the encounter on the boat i work on it was rough seas and i had nite sweats that nite for a couple of hours and got pain in the left side of my pelvis that lasted a few days, I get motion sickness bad. I haven't had and fevers or diarrhea or vomiting or Nausea. I have psoriasis and it cleared up on my hand about a month after the encounter . 1 can I trust my test results at 8 weeks? 2, can all the symptoms I've had be from stress? My Stress level has been to the point of Suicidal. Any thing will help. Thanks so much.

I have gone through the details and can make out your anxiety.
The chances of transmission of HIV by unprotected intercourse with a HIV positive person is 1/200 to 1/1000.It is too low.
Not all intercourses can lead to STDs or hepatitis or syphilis.
They may be negative too.Even the sex workers will be cautious and would repeatedly undergo tests and treatment.
Tests like syphilis,chlamydia,gonorrhea,are conclusive by 8 weeks.You can allay fears.
It is good that HIV, hepatitisB& C are negative at 8 weeks. It is better you undergo these tests again after 12 weeks to get conclusive results.Though the chances are less to come positive later it is better to go for tests.
Just ward of all your suicidal tendencies and anxiety. The chances of getting STDs is less.Moreover All STDs can be managed.In future follow safety precautions.
The symptoms of thrush can be due to fungal infection, smoking,other infections. It is not specific to HIV.
Penile dots are not specific to HIV.
Weight loss may be due to anxiety or stress.
Take protein rich balanced diet.
Do exercises, meditation, yoga.Get involved in activities of your interest.

Thanks DR Bharatesh. i have a couple more questions. with the pelvic pain i mentioned that came on to days after my encounter i have had red bllod in my stool that did not come from anything external what could that be and i will be testing again for HIV would a Oriquick at home test be conclusive at 14 weeks. and the RPR at 8 weeks is absolutely conclusive? Thanks so much DR.

Thank you for follow up query.

The RPR tests would be conclusive .You can repeat it after 12 weeks too.
The blood in stools if persistent needs evaluation. Otherwise need not worry.If temporary have a high fibre rich food with stool softeners.They are not specific to HIV.
Antibodies test like Elisa is preferred as conclusive test over oraquick test though helpful.
