
What Do These Chest X-ray And Abdominal Ultrasound Reports Indicate?

I have multiple gland in my neck( recently observed), doctor has diagonised as "Multiple Gland Swelling".Various test has been completed like:
USG OF THE WHOLE ABDOMEN :
Comment of the this test :"Sonologically mild fatty liver"
CHEST X-RAY PA VIEW :COMMENT : "Lung fields expanded.Veil opacity right apical region- ? infective pleural thickening.Central mediastinum with normal cardiac size.
Domes of diaphragm and adjoining angles are normal.
Suggested follow up study."
Blood test of HB%/TC/DC/ESR/FBS/TSH/FT4 are done.all blood reports are within normal range except:
ESR by EDTA Blood(method: Westergren,manual ESR Tube) found 22mm/hour
TSH3-Ultra(method: CLIA) found 6.05ulU/ml
Present medication from 28th april,2016:
1. Azithromycin tablet 500mg one before dinner (for 12 days)
2. Tab Thyroup 12.5mg in morning
3.omeprazole capsule 20mg once in morning
4.cap wellman one after breakfast
I am feeling tired and less oxygen to breath.
I don't have any soft copy of my reports and prescription for upload thus everything has been typed above, Please suggest me what should i do, should i consult any chest specialist? sir please suggest me.

Thanks for posting in HCM.
I went trough your query and understood your problem.
I feel it could be tuberculosis which has not been investigated properly enough.
Three things in favour of TB are-
1. Cervical lymphadenopathy (most common reason being TB)
2. Raised ESR
3. Apical opacity which again points towards TB.
These are pretty much suggestive points for tuberculosis.
I would advise you to consult chest specialist and do not ignore it. At least get the investigations done, if it comes negative well and good. But if it comes positive, you will need to undertake anti tuberculosis treatment.

I am a guest lecturer of a college where I have to take classes(full session will start from july,now invigilation duty of exam etc going on i.e less speaking work now) and I also teach students in home where I have to speak for 2 hrs continuously(weekly 2 days),sir I have some quiries,please sir,please answer these questions:

3.Sir I am unmarried,my marriage has been decided (preliminary decision taken) to be held in FEB,2017, If TB found should it be postponed?
4. Is there any restriction of consuming food?
5. I have in my home grandmother(80+),father(63),mother(50). elder brother out of station. what are the precautions to be taken by me & or us?
6. If found TB how long I have to confined to home/ take rest?

Thanks for following up with me.
Tuberculosis is a bacterial infection. There is no connection of drinking or smoking with it. Its just like your any other infection and it s curable.
I will surely answer all your queries. Here are the answers-
1. Yes it can spread through your cough as it harbors in your sputum. But I am not sure here whether you have pulmonary tb or TB lymhadenitis.
If its TB lymhadenitis then you won't be spreading any disease. But if its pulmonary and you are coughing then you might be transmitting the disease. So it depends if you have cough or not.
2. Additional tests will be sputum examination for AFB culture if you have cough. TB PCR is very sensitive test but bit expensive one. If you do not have cough then FNAC of the lymph nodes in neck will be required. That means a needle will be pricked in the swelling under ultrasound guidance and some sample will be collected for examination. These tests will be enough to diagnose whether you really have TB or not.
3. No need to postpone. It is totally curable. Starting the treatment and complying with it is very important. You will be fine with it. NOTE- ALL THIS IF IT IS DIAGNOSED ONLY.
4. No food restrictions as such. Your usual diet can be taken.
5. For now if you have cough, use mask. Your cough if at all present contains the bacilli for TB. If they are inhaled by others then can get infection. If you do not have cough then you won't be spreading it.
6. No need to be confined. Again it depends whether you have cough or not. If you have cough and it is surely diagnosed to be TB, few days like 1 week you need to stay away when treatment is started. Then you can resume your daily routine after that but treatment should be continued.
See XXXX, there are 2 things.
1 is you are not yet confirmed to gave TB. I can be wrong also. After relative tests only it can be confirmed.
2nd is I do not know whether you have cough or not with sputum production hence I have answered with both possibilities above in my answer.
Kindly consult a chest physician asap and get the above tests done.

2.FNAC from Cervical Lymph node
Suspected disease:
TB OR LYMPHOMA
Medication:
1. Tab Azithral(500)--ODAC For 6 days(already started from 28/04/16 i.e continue upto 03/05/16 )
2.Cap Nexpro-BD (40)----1 Cap ODAC to cont..
said Cap WELLMAN may be continued.
For your info :
1. I don't have any cough/fever.
2. I have some glands visible in joint/starting part of low waist and upper leg.
Sir tomorrow I will do the tests.
Sir if I have TB then it will be curable but if it will be LYMPHOMA, then what will happen sir???? please help me, please suggest me sir? please suggest...

See XXXX, we cannot predict and keep on guessing. You get the reports done as that doctor have mentioned.
Good you don't have cough so you are not transmitting the disease.
You please continue all the meds as prescribed by your doctor.
We will wait for the reports to come and then take decision as to what can be done.
This new inguinal lymhadenopathy is a new finding and its better if we do the fnac and mantoux test too.
Once diagnosis is confirmed further steps can be taken.
