Detailed Answer:
Hello! Thanks for putting your query in HCM. I am a Gastroenterologist (DM).
I appreciate your concerns regarding your brother.
I have gone through all the reports you have uploaded.
Endoscopy examination is clearly suggestive of reflux oesophagitis with patulous lower oesophageal sphimcter. Due to patuous sphincter the symptoms in him is severe and leading to vomittig, burning sensation, loss of weight and apetite and increase of saliva.
For this I will suggest following which I found very useful in my patients with reflux oesophagitis.
1.Avoid fatty and spicy meals and citrus fruits
2. Don’t lie down soon after meals for at least two hour
3.Take small and frequent meals, instead of full meals at a time
4. wear loose clothes
5. If he is overweight or obese do weight reduction with dieting and exercise
Among medications:
1.Take ilaprazole which is a proton pump inhibitor, it is most efficacious and with minimal side effects presently available along with a prokinetic agent like levosulpiride before breakfast
2. For more better results ask him to take tablet famotidine after dinner
3. If in between symptoms ocurr then may take antacid containing oxetacaine which is a local anaesthetic like mucaine gel
One more thing stool examination is showing presence of giardia lambia, for which take ornidazole for 7 days
Usg abdomen suggestive of liver hemangioma which is a benign condition so there is nothing to worry about that
I hope I have answered your query and this will help him. If you have any further query I will be happy to answer that too. Remain in touch and get-well soon.
