What Do These Following Lab Reports Indicate?

Question : Hello Doctor,
How are you, Hope your doing great.

In the previous discussion had with you, to debrief " I had protected Vaginal sex and unprotected oral sex with 3 girls 25 days back and was suffering with severe trauma and anxiety to get my doubts cleared.

And as advised I finally got tested for the following tests.
STD Panel test from Lalpath Lab which includes below:

1.     TPHA; TREPONEMA PALLIDUM HEMAGGLUTINATION
2.     CHLAMYDIA IgG ANTIBODY PANEL- CHLAMYDIA TRACHOMATIS/CHLAMYDIA PNEUMONI
3.     HERPES SIMPLEX VIRUS (HSV)1 +2, IGM, SERUM
4.     HERPES SIMPLEX VIRUS (HSV) 1+2, IGG, SERUM
5.     HIV 1&2 ANTIBODIES, SCREENING TEST INCLUDES P24 ANTIGEN
6.     VDRL (RPR),SERUM

Also in another diagnostic center, I got tested for :
7.     HIV 1 &2 Antibodies- ELISA Method
8.     HIV PCR-DNA Test
I ve taken the screen shot of all the test results and attached in two pdf documents, please find the attached reports and give your valuable feedback.

Regarding HIV all the three reports is negative, can these reports be considered conclusive at 25 days of risk exposure?

Regarding STD Panel test they took only blood sample for all the above tests mentioned, they did not take any urine sample?

Based on the above reports am I effected with any STD? Also do I need to get tested for Gonnohrea?

All the above STD test at 25 days of risk exposure are conclusive?

Can I continue having sex with my wife? is it safe now.

Welcome back to Health Care Magic.
I have gone through the details and reports.

The tests for STDs (syphilis, chlamydia)are negative.HSV IGG positive, indicates it is a past infection.It may be due to past herpes infection(oral or genital herpes). It may be due to oral herpes too.It depends on your symptoms or lesions earlier.

HIV Elisa, P24 Ag,DNA PCR tests are negative after 25 days.It can be considered almost conclusive . The chances of HIV tests coming positive later is almost nil.Results are considered conclusive after 3 months.

You can go for Gonococcal tests if any symptoms.
Chlamydia positivity is not due to STDs. It may be due to respiratory infection for which you have already taken azithromycin.

The chances of transmission of HIV to your wife is very remote.If it is genital herpes ,you may have to avoid intercourse especially when there are active lesions.You both can decide on having intercourse.

Thank you doctor for your feedback. However I would like to seek your advise further.

You said Hiv elisa, P24 antigen, Hiv dna pcr are negative and almost conclusive. Do I need to take the test again or not required. Is there any chance of false negative?

Also regarding Herpes it showed positive and you said it might be due to previous infection. Is it due to STD? I don't have any lesions on penis, however I have mild itching under the skin of testicle area.

Regarding chlamydia is there any infection based on the report? Do I need to take any medicine.

Again do I need to get tested for Std?

Thank you for follow up query.
I am sorry for the delay due to technical reasons.

HIV results are almost conclusive. The chances of tests coming positive later is nil.It may be better to get tested for HIV Antibodies after 3 months to be totally relieved of anxiety.All 3 tests cannot give false negative results.

Herpes can be oral or genital lesions.If you can remember any symptoms (grouped water filled lesions around oral or genital regions with itching or pain or burning sensation), it is likely due to it.
Some may be asymptomatic too.It may be transmitted as STDs or even by skin to skin contact or kiss too.

If you do not have any symptoms you need not worry of chlamydia as you have been treated with azithromycin already.

You can get tested for STDs and HIV after 3 months as a routine check up.

Thanks for the response doctor, you also mentioned regarding Gonococcal tests if any symptoms.

What are these, and what could be the symptoms? if there are no symptoms still i need to check.

Also since you are saying that it is better to get tested after 3 months, shall i refrain having sex with my wife till then?

Also after 3 months of exposure shall i get tested for HIV Elisa test, is it sufficient? Also what STD tests you recommend as a routine check up after 3 months.?

Please advise.

HIV tests Elisa for HIV 1&2 antibodies would be sufficient.
Gonococcal infection symptoms will be pus or white discharge from genitals with burning urination .
You can get STDs panel tests including HIV tests,chlamydia tests,syphilis, HSV 1&2 antibodies, gonococcal etc.after 3 months.

You can use barrier methods like condom .

Ok doctor, They can be considered conclusive after 3 months of exposure for both HIV and STD.

No further testing is required.

Also can you please throw some light on gonoccal tests, what is the test called? what sample do they take? In which diagnostic centers the test is done.

Yes it can be considered conclusive after 3 months.
Gonococcal tests are usually done by taking smears from urethra,(or urine ) for staining, culture,PCR tests.Serology for antibodies may be done in later stages.
They will do it any of the laboratories like XXXXXXX lalpath,Ranbaxy, etc

Thanks Doctor for the prompt response.

But I have a last question, I got tested for HIV-Elisa, P24 antigen , HIV PCR DNA.

and for STD's (Syphilis, Herpes, chlamydia). But for Gonnoreah I have not done any testing. Is it possible that i do not have any symptoms and have Gonnoreah?

Do you suggest to get tested for Gonnoreah as well?

It is now 30 days post exposure??

After 3 months as advised I shall get tested for HIV-elisa for conclusive result and also I need to get tested for all the STD tests?

Please advise.

You can HIV tested after 3 months.Other STDs may not be necessary. If anxious you can get them done or if you develop any symptoms during the said interval.
If you do not have symptoms I do not see the need for gonorrhea tests now,though some may have asymptomatic infections. The results also is likely to come negative.

Hello Doctor,

Just one clarification, I have been using antibiotics since last one week as I ha mild throat pain. it is almost a week still i see mild irritation on throat and numbness on the jaws.

I am getting tensed. Though I got tested for HIV PCR-DNA ,Elisa and Antigen after 25 days and all are neagative, I am worried if i have done the tests below the window period.

I am getting worried. Do you recommend to test again after 6 weeks of exposure.

Because initially after one week of exposure i had Ajithromycin for 3 days and alter after 25 days of exposure iam having antibiotics for ciproflaxin.\

Please advise doctor i am getting worried.

Also PCR-DNA test is better or PCR-RNA? is PCR-DNA conclusiv after 25 days?

I have gone through the details.

HIV RNA PCR and DNA PCR have similar sensitivity after 2 weeks post exposure. The tests have been done around 25 days is almost conclusive.These tests are the best tests during window period.
Antibiotics do not alter the test results. So you can allay fears of HIV.
The symptoms are nonspecific and are not related to HIV.
There is no need for further HIV tests until 3 months.After 3 months just an antibodies test will relieve your anxiety totally.
Relax,forget the episode. Do yoga,meditation, exercises to keep fit.Get involved in activities of your interest.
