Question: Sir

My SGPT & SGOT is little bit on higher side.I like more non- veg foods like chicken;KFC chicken.

The problem started with very frequent loose motion & diarrhoea.Upon lft test it was come to know that SGPT & SGOT is on higher side.I am suffering from depression & used to take venlafaxine -37.5 mg; Betacap TR 40 mg & lonazep me 0.5 so.

I am gradually coming out of depression & medicine.

At present my problem is high sgpt & shot.

Done so many test.report attached.

As per your queries answers are-

Minimum 3 times elevation of liver enzymes are necessary to have any clinical significance.Mild elevation of liver functions have no clinical significance.
Enzymes are produced by liver cells and a part of it diffuses into blood stream.
Permeability of liver cell for these enzymes varies day to day(slightly) which depends on food we eat, medications etc.The medication which you are on are metabolized by liver..Continuous use of medications alters the permeability of liver cells and may cause enzyme to leak into blood stream temporarily.
So take home message is-
Your LFT is only mildly elevated.
Mild elevation of enzymes have no clinical significance.
Medications can cause temporary swelling in liver cells resulting in increased cell permeability hence causing mild elevation in blood enzymes.

-Advice take a liver rejenuvator like sylimarin once daily,It will help to rejenuvate your liver or take LIV52 once daily.

As of now there is no need to worry.

Dear sir
Thank for reply.
At present the doctor under which I am undergoing treatment wants to start medicine for diabetes.HbA1C -6.0.He wants to start medicine but I rarely eat sweets.He is also advising for colonoscopy.
More over I want to state that so many reports of LFT are uploaded & report show many fluctuations & increase in SGPT & SGOT.

I also feel that his diagnosis is not appropriate so I required 2nd opinion.

Hello again dear,
As per standard text.
The normal range of values for AST (SGOT) is from 5 to 40 units per liter of serum (the liquid part of the blood). The normal range of values for ALT (SGPT) is from 7 to 56 units per liter of serum.


So going by the above mentioned value to be significant the levels of AST should be at least 120 units/litre while levels of ALT should be atleast 150 unit/litres.
As you can see by your self the above mentioned data is in safe limits and increase in enzymes are most likely due to the medicines you are on.
You should start taking Sylimarin 2tsf once daily
Liv 52 tab once daily for 21 days.
It will help your liver to counter the effects of medications you are on.


However to be on safe side get a ultrasound upper abdomen done.If its normal there is no need to worry.

Thank you for prompt reply.
I would to like to ask that you have not put any comments on sugar test report as I mentioned regarding Hbaic & Sugar report.my doctor wants to start medicine for sugar.
At present I am taking Adliv fort & Evion- 400 mg.Should I continue?
Please advise regarding foods to take.Doctor have advised to take less fatt & more protein diet.
Are Venlafaxine-37.5 mg & Betacap TR-40 ; Clonazepam increasing LFT report.should I ask doctor to change medicine for depression as liver enzymes are changing.

Hello again my dear patient.
Yes you should be started on low doses of metformin(metformin 500mg once daily).HbA1c is suggestive of long term control of blood sugar however in your case your mean sugar is deranged therefore initially therapy with metformin should be started for 3 months along with drastic life style modifications and food restrictions. Hba1c would be used thereafter to monitor the effect of medication in long runs.

Some food plans are=
-Whole grains are very beneficial to consume.These can be in the form of XXXXXXX whole wheat bread or cereal, brown rice, whole grain pasta,grains such as whole oats, wild rice, rye, oatmeal and corn are also beneficial and can be taken if suits your food pattern.
-Fruits and vegetables should be a significant part of any diet to help in recovering from a liver disease. They are full of essential nutrients and are easy to digest.When on recovery diet follow a high carbohydrate diet,Fat contents of diet should be very low.However Olive oil, canola oil and flaxseed oil are all healthy fats that are recommended as part of a diet for patients with Liver problems.
-Healthy proteins in the form of low-fat milk and dairy products along with lean meats, beans, eggs and soy products can also be a part of a healthy liver diet.

Now lets move on to
Foods to Avoid
-Processed food items
-Hydrogenated oils
-Avoid alcohol and smoking.

Are Venlafaxine-37.5 mg & Betacap TR-40 ; Clonazepam increasing LFT-Yes all these are directly metabolized by liver.Unfortunately whether its Tricyclic anti depressant or SSRI or SNRI all these groups directly or indirectly are metabolized by liver.So doctor does not have many options and your liver enzymes are not elevated to an extent that you need to switch over to other medicine.As suggested above include antioxidants and sylimarin in your regimen and parameters would improve in few weeks.
