What Do These Ultrasonic And Blood Test Reports Indicate?

Question: Hello Doctor

i 25 year male from XXXXXXX
For the last one month i have been experiencing somekind of difficulty in my throat. i feel some thing there, especially after lying sideways. I visited a doctor and took Ultrasonic test of my neck and Blood test yesterday. Results are attached.

According to him i have MULTINODULAR THYROID. Doctor told me to wait for 7 days. Then he will me whether to visit a surgeon or not. I have GERD and allergy(dust and smell of perfumes).

1 month ago when this problem started doctor told me to do some tests like- SGPT,Fasting blood glucose,cholesterol,
Blood routine - haemoglobin and Total count,
Differential count - polymorph,lymphocytes,Eosinophils,ESR,AEC,
Albumin,Sugar,
Microscopic Examination - Pus cells,RBC,Epithelial cells,cast,crystals,bacteria
and doctor said everything is normal.

I am really worried now. Is it a serious disease?. Will i be able to lead a normal life in future?. What will be the treatment and how long i need to take it. Is it a curable disease. What are the other problem associated with this disease.

Hello, I really appreciate your concern. I am Dr. Manan Patel from XXXXXXX answering your question.

Multinodular thyroid gland is a benign disease which is very common in population. It goes unnoticed until there are any signs or symptoms. It is a non dangerous condition so you can definitely able to live a normal life.

There is no specific reason for development of this condition. It can be due to some genetic mutation or iodine deficiency. No specific treatment is advised until there are symptoms of hyperthyroidism or difficulty in swallowing or disfigurement of body.

Your TSH report is normal so in that case no further treatment is advised at present. You should get Thyroid hormone profile done at regular intervals.

In some case there is increase in levels of thyroid hormone which leads to symptoms of hyperthyroidism like palpitations, heat intolerance, muscle weakness, hyperactivity, tremor, irritability, weight loss, increases appetite etc. In such cases anti-thyroid treatment or radio ablative procedure can be advised. So it is a totally curable disease. The duration of the therapy varies from person to person according to decrease in size of nodules.

Treatment prescribed to you is for GERD (Gastro Esophageal Reflux Disease). This is probably the cause of your sensation in the throat. Continue taking those medicines for relief of symptoms.

So i need not to worry about it?

At what regular interval should i take thyroid hormone profile? and how long should i continue doing so?
If it get worse is it 100% curable? Will it affect my work and life at that stage?
Is surgery only option at that time?

My doctor told me after 7 days he will advise me whether to consult the surgeon or not.If there is no need to worry at present then what might be the reason?


next week i have a medical test. Do i need to worry about Thyroid profile? Will there be any big change in 1/2 week?


I will have to join my new job on september last week. I will be on probation for 2 years. I will be posted far from my home. So the things like surgery, chance of present situation becoming worse make me afraid.

Hi dear,

There is no definite guidelines for thyroid profile. You should have thyroid profile at least once in a year.

Without evidence of rapid growth, obstructive symptoms (e.g. difficulty in swallowing, cough, shortness of breath), or thyrotoxicosis symptoms as mentioned by me, no treatment is necessary.

There are three main treatment options for this condition Surgery, Radio active iodine therapy and levothyroxine (T4) therapy. This condition is 100% curable by all the treatment approach. It will not significantly affect your work at any stage of life.

Your doctor might have asked for surgeon opinion to rule out any compression of airway and to assess any need for biopsy. At this stage you are not having symptoms so there is no need of biopsy.

Your medical test for new job will not be affected by your condition. Your TSH level is normal. So don't be afraid. My best wishes for your new job.

Just one more question.

I have been taking these medicines for the last 3 days. Still i have same problem with my throat. it is like, some thing stuck there. i feel it especially after lying sideways. Some time it doesn't feel good if i have not taken any food or water for a while.

Hello,

The feeling of something stuck in the throat could be due to thyroid nodule compressing your esophagus when you sleep sideways. As per my opinion surgery is not necessary in such case. If there is difficulty in swallowing food then you should consult a surgeon for definite treatment.
