What Does ANA Titre Of 6400 Indicate?

Question: I would like your opinion on my recent blood results for autoimmune. Antinuclear antibody positive. ANA titre 6400. ANa pattern- multiple nuclear dots. SMA positive. SMA antibodies positive( weak) . I have B12 deficiency. Just recovered from kidney infection. Blood in urine since April. I never had a kidney infection in my life. Getting B12 injections for the last 2 years. Bloods have been reading this for the last 14 years!!!

Thank you for writing on health care magic

Well your ANA is very strongly positive, but since you are saying it has been there for 14 years it may not be that relevant as 5 - 10 % of normal population can have ANA +ve.

But the titres are very high and i would wanna ask you a few questions as blood in urine does sound a bit alarming

1. Could you send me a detailed report for the Urine. I want to see if there is presence of proteins or casts in it

2. Do you experience any symptoms like : Oral ulcers, Rash over the face or arms, Photosensitivity,Excessive hair fall, joint pains

3. Did you ever experience any bluish discoloration in the fingers during the winter season or are you experiencing dryness in the mouth or eyes

4. Have you ever got tested for ENA Profile, Complement levels and Anti Ds DNA

5. How about the other blood reports: ESR, Kidney Parameters, Thyroid Levels and Liver function tests

6. Any other change which you have been experiencing

ANA can be positive in 5-10 % population and even in thyroid disease but there is a chance that the person may develop autoimmune problem during it s course.
So do let me know these queries and i will be able to guide you further...

Hi here are some more blood results. Total protein- 70. Albumin 41. Bilirubin 4.7. Alkaline phosphatase 83. Gamma-glutamyl transferase 64. Alt 30. Urea 1.8. Creatinine 56. Thyroid stim.hormone 1.17. Free T4 18.4. Urine culture-colony count - no growth. Hb 13.5. Pcv 0.404. Mcv 97.4. Mch 32.5. Mchc 33.3. Hdw 1.95. Platelet 310. Neut 6.57. Lymph 1.28. Mono 0.34. Eos 0. 07. Baso 0.09. Luc 0.06. Esr. 23. Wbc 8.41. Rbc 4.41. I am suffering dry mouth for the last 8 weeks. Mouth ulcers. I had one episode when the tops of my left hand fingers went blue. I have slight rash on both arms. Had photosensitivity in XXXXXXX My ankles and arms are swollen since XXXXXXX but my gp doesnt seem too worried about them!!! I never had them before and i think they look terrible. Ana pattern- multiple nuclear dots.

Good evening. Sorry for late reply as it was night time in our country. First thing I am doing after waking up is answering your query.

I have gone through your blood reports and history. Rashes, swollen ankles and hands, Ana positivity, dry mouth all point towards a connective tissue disease. To find out which one is sort of difficult without the further set of antibodies as lot of these diseases have quite the same symptoms

I do want to see your urine routine report still to see then blood and protein loss in it. I would suggest you to schedule an appointment with a rheumatologist there, so he can have a look at your condition from a different angle.

Meanwhile you need to get tested for ENA profile and complement C3 C4. ENA profile checks another 25,antibodies of Ana and helps point us to the connective tissue disease if any.

My last query is:

1. is your sleeping pattern OK,

2. do you experience any irritability or forgetfulness as well

History noted:

At this stage, since I have not examined you and from the blood reports and history you have told, I would suggest you to get the following test done:

1. ENA Profile
2. Complement C3 and C4
3. Schirmmers Test( Strip test for eye dryness)
4.X Ray Chest PA View
5. Repeat your urine routine once again..

It will help us give a clue to which connective tissue disease we are dealing with. Once you have these results , you should schedule an appointment with a Rheumatologist. It will be easier for him to give you the right diagnosis and the treatment in the first go.

But if these results come back negative, there is a less reason to worry about as there is nothing serious. ANA could just be positive but not causing any harm
