Brief Answer:
He requires further evaluation.

Detailed Answer:
Hello dear. I have gone through the details.

So a rising PSA despite surgery and radiation means the tumor has most likely recurred. He will now require an evaluation in the form of an MRI and bone scan to look for any disease relapse.

Moreover, the additional treatment in the form of medical or surgical castration will be required.

Any nearby center having a genitourinary cancer doctor will be fine. He still has a good amount of years to live.

Thanks and regards
