Thanks for your question,
Though your Fasting & PP blood glucose showed fair control, HbA1c is on higher side.
Considering your small dose of first line drug (Metformin, content of Tab Glycomet) for diabetes there is very good chance of achieving much better blood glucose control.
I would like to advise-
- Follow diabetic diet.
- Regular exercise approximately 150 min per week of moderate degree of physical exercise like simple brisk walking, according to your tolerance level.
- In crease the dose of metformin to 1500 to 2000 mg per day in divided dose.
Tab Glycomet 850MG twice a day may be a good option, if there is no issues of heart and kidney problem.
- Start Tab containing Atorvastatin (lipid lowering drug) dose will be decided on the basis of lipid profile.
Check Blood Glucose Levels (F & PP) after 4 to 6 weeks, accordingly appropriate modification in medicines can be made.
You should discuss all this with your treating diabetologist.

* Answer to First 3 questions-
- Metformin is the first line drug in the treatment of Type 2 Dibaetes, and is accepted world wide.
- It has minimal side effects. Some stomach upset in the initial period, there after body adopt to it. Hence usually started in small dose with gradual increase in dose to maximal over 2 to 3 weeks.
- Metformin decreases the insulin resistance which is the important cause for Type2 diabetes. Usual effective dose of metformin is 1500 to 2000 mg per day for maximal effect.
- So it is advisable to take at least take 1500 mg of metformin per day.
- Diet and exercise are the most important steps in the treatment of diabetes but it may not be possible to control blood glucose level every time with them, so optimal dose of metformin is advisable with them in your case.
* 4 * Use of insulin early in the treatment of diabetes may be considered but only after failure of optimal dose of metformin to control blood glucose level.
* 5* Alcohol maximum up to 2 drinks (pegs) per day can be accepted.
*6 * Increased homocystein may associated with increased chances of development of blockages in blood vessels this risk increases further with uncontrolled diabetes, blood pressure, lipid profile, habits like smoking.
It is usual practice to use Statins (Atorvastatin/ Rosuvastatin) in all diabetic patients after the age of 35 yrs irrespective of Lipid levels. Only dose will be decided depending on lipid level.
So you should continue tab Nevz-B and start any statin in minimal dose if lipid profile is normal, after discussing with your doctor.

1) Whatever you have mentioned are company names that contain drug Metformin only or in combination with other antidiabetic medicine.
2) You can use any company formulation that contain only Metformin in sustained release form.
3) It depends upon formulation.
Formulation containing sustained release form of Medicine can be taken once or twice depending upon dose and that contain plain form has to be taken more frequently.
4), 5) Dose of Metformin depends upon blood glucose level, side effect profile, Kidney, heart & Liver function.
As duration of diabetes increases, there may be deterioration of liver, Kidney, heart status, also depending upon blood sugar levels appropriate dose modification in Metformin dose need to be done.
It is usual with diabetes that as duration of diabetes increases there may need of increase in dose of a drug or addition of one or more new drugs to control blood sugar levels.
6) You should restrict calories and fat, increase intake of protein, salads.
You should consult nutrition specialist who can plan diet for you according to your BMI and food habits.
