Brief Answer:
Ultrasonography ASAP.

Detailed Answer:
HI.
Thanks for your query.

I would like to know whether the growth is just under/ on the skin of the scrotum
Or it is on the testicle proper ( free off the skin of scrotum)

If the growth is from the skin- attached to the skin, elevated above the skin surface, it is probably a sebaceous cyst. A small Surgery of removal of the cyst will solve the problem.

If the swelling is under and free of the skin , then only it is possible that it is from internal structures.
The possibilities according to findings are::
If the swelling is rounded, smooth , painless- it can be a hydrocele or a mass of the testis.

Now the most important point is Hydrocele is just a collection of fluid and a benign condition- not threatening. BUT a mass of the testis can be a cancer- life threatening condition. Unfortunately both are painless.

So your hubby has to undergo an urgent ultrasonography or MRI to know the diagnosis so that further treatment can be planned.
