Question : My wife had medical abortion on 10th XXXXXXX After review she was asked to take another two pills and do UPT. But she forgot to take those medicine(misoprost) and did her UPT which shown positive pregnancny. Doctor is now doubting infection because of this. Please suggest what has exactly happened and best way forward.

she has started her period but which is very irregular and low quantity of flow.

In my opinion, first she should go for an ultrasound pelvis to see whether abortion is completed or not.

Pregnancy test can be positive after abortion for upto one and half month.

So please do not take stress and please send your ultrasound report as early as possible.

After ultrasound report, it is confirmed whether she requires D&C ( surgical abortion) or not.

she feel very week thesedays. Is there is any longterm health risk due to this? Is there much to worry about this situation now.

Sorry for the delay in my response.

Your report was not showing on my desktop during the night, now I am able to see it.

I have gone through her report and her ultrasound showing that there is some fluid collection in her uterine cavity.

In my view, it can be due to retained product of conception, fibroid or due to polyp.

Two more thing, is there in her ultrasound one is fibroid and other is polycystic ovaries.

But the most important thing to rule out the cause of fluid in the uterine cavity, so she should go for repeat serial beta HCG level and sonohysteroscopy to rule out the cause.

So are we trying to rule out pregnancy through HCG test. Does her ultrasound shows any problem requiring D&C? I am still not sure. We have already given blood for HCG test.

Beta HCG levels are done to rule out three causes-

- Mildly elevated in Retained product of conception. (RPOC).
- Highly elevated in Gestational trophoblastic disease.
- Normal in uterine A-V malformation.

If RPOC is there than D&C require in her case.

She is still bleeding after abortion that is likewise a sign in favor of RPOC.

after taking abortion pills she bleed for 6 days and then stopped. Now her bleeding started because of her expected period date although its not much and irregular. Infact bleeding stoped today again.
