Question: Is a rise in cea value from 3 to 4 in 1 month concerning in a person who had bowel cancer surgery in march?

No. This is not significant value and cea values fluctuate from month to month even in healthy people and this is especially seen in early postoperative period. You should just repeat cea in2-3 months and if there is more than double elevation then you might need to do pet scan or colonoscopy. He was a dukes c1 and has just started oral chemo tabs a week ago as initially he was refusing chemo.

Is it advisable to complete the 6 month chemo course full?

Definitely yes for dukes c. for dukes b it may be questionable but for dukes c there is no question about necessity of full chemo course. Chemo is always unpleasant but trust me it will raise chances of long time survival and this is only way to try to prevent recurence. Prognosis depends on number of positive lymph nodes but with radical surgery and chemo more than 50% of patient reach 5 year survival.

Yes, it is good sign that only local nodes are affected. 10 is bit above average number of affected nodes which is seen in dukes c and it means that chemoth is required but you can still hope for good prognosis( 25-30% will survive for 5 years).
