Appears to be allergic dermatitis over penile area
Detailed Answer:
Hello young fellow,
Welcome again to the forum and thanks for posting the important details. After analyzing the complete situation I can make out that you may be suffering from the allergic dermatitis over penile area.
To tell you simply- our skin is renewed regularly and there is barrier function of the skin by which the things coming in contact of our skin do not cause any problem in majority of cases. But at certain times when this barrier is breached then it leads to entrance of all the things into the skin rather than staying superficially. Most importantly the soaps and detergents are commonest cause of irritants to the skin. This irritation is manifested in form of dry, red skin or torn and ragged skin on penile head.
Even many time the undergarments which we wear may have detergent particles in the clothes. So the tip of penis coming in contact may get irritated and red.
As per the history as well as picture it does not seems to be related to any sexually transmitted infection.
I would like to provide you certain suggestions which would be very helpful in reducing the present symptoms and in future also:
1. Do not touch the penile skin too often.
2. As you are uncircumcised male so clean the genital area 3-4 times thoroughly and gently by retracting your prepuce completely backwards.
3. Do mild warm water compression meaning cotton soaked in the water and compressing for 5 minutes.
4. Stop application of any kind of medication without the advice of the doctor. Even do not apply multiple kinds of antifungal and steroids which itself can lead to sensitization of the area.
5. Avoid intercourse as well masturbation for few weeks (atleast 2-3 weeks). After every sexual intercourse too, wash the area properly.
6. Wear cotton undergarments. Change these two times a day. Do not wear any synthetic or tight garments. Wash your undergarments by properly rising in adequate amount of water.
Again a final word of suggestion is not to touch the area unnecessarily any time. We quite often touch the penile area in anxiety and therefore irritate the skin which is one of the sole most important cause of penile redness.
Lastly kindly reassure yourself for this simple problem which can be taken care of by simple meticulous methods.
I hope these information's will help you. If you have further queries I will be happy to help or if not then can close the discussion and rate the answer.
"Wish you good health"

I have attached for your information what the rash looked like a couple of months ago so i think it has got better.
Can i ask what you think the white spots i had on there do you think were?
How long for the lesion do heal fully?
How do i do the compressions?
Previous picture show mild excoriation & redness
Detailed Answer:
Hello young man,
Thanks for your follow up.
I have analyzed your previous picture and can see that the redness was quite high previously.
Thanks a lot for the very good resolution picture that I could analyze the picture at higher resolution. You can notice that the whitish area is mild superficial peeling of the skin. The red aera is actually gradually replaced by the very superficial erosion. It is quite a common phenomenon due to thinnest skin of penile area. Nowhere else in our body has such a thin skin that even the blood vessels are directly visible over the surface.
Presently the redness has been replaced by mild excoriation and erosion at two places. These erosion are common observation if the area is touched too frequently. I quite often counsel your kind of cases and in most of the cases it is the anxiety or fear of something because of which we touch or try to see the area too often.
That is why I suggested you to not to touch or see the area too frequently as it may worsen the situation.
If you do not touch area unnecessarily then it will heal in next 7-14 days by itself.
For mild warm compressions- Take a glass of plane water and you can add a pinch of salt in it or otherwise you can take normal saline water from your local pharmacy. Dip a sterile cotton ball in it and compress it for 1 minute. Do it for around 5 minutes for 3-4 times a day. It will help in healing as well as reduce the accumulation of bacteria and other allergens causing the problem.
Based on the complete history I again reassure you that the condition does not seems to be any Sexually transmitted disease. Therefore do not get tensed about same.
I also suggest you to always uses protection in form of condom in every sexual act. Safer sex is always better than unnecessary tensions later.
My all best wishes for early recovery of the problem.
I will be glad if you need any further clarification or if not then can close the discussion and rate the answer.
