What Does A Small Red Spot On The Leg Indicate?

Question: Dear Doctor,

Initially (almost 3 -4 years before) i had small red spot in my mid of leg. I thought to treat it with fusidine cream which ended up with more brown spots in my leg. I have consulted with so many doctors, they have not identified this issue instead of applying their cream the discoloration of my skin got spreaded more.
Finally, I have met a skin specialist he told its thrombosis. The skin specialist recommended to take trental for 6 months (first 3 months morning-night and next 3 months only night) , i have taken almost 3 months but I don't see any positive results in it. Then skin specialist had also done TCU peel on my leg (2 times) whereas no effect from this peel rather the discoloration got more darker.


Moreover, I have consulted with vein specialist, after Doppler scan he said everything is normal. I am an IT guy, who has to sit and work for long time.
Also, if I do exercise or travel more distance, again my skin is getting more reddish spot.


The second thing, I wanted to take Omega 3(1000mg) for memory and cognitive function, if I take omega3 in morning and pentoxifylline in night will it have any drug interaction.


I am fed up with this all treatment, I am looking for advance treatment which could solve my problem. Attached picture of my leg and doppler scan results

I am dr.kakkar (dermatologist). I have gone through your query and I have viewed the image.
You have a condition known as Schamberg's purpura. In this condition there are reddish brown patches commonly on lower legs and ankles due to leaky capillaries. The appearance of these patches resembles "cayenne pepper" and is due to breakdown of red blood cells and deposition of hemosiderin pigment.
Unfortunately there is no treatment which is completely effective however topical steroids have been used and have shown a good response in patients. Other treatment options include phototherapy and oral pentoxifylline (trental)
I suggest you to use a moderately potent topical steroid cream e.g betamethasone valerate 0.1% cream (betnovate cream) or mometasone furoate 0.1% cream (momate-XL cream), twice daily for 3-4 weeks.
You can continue with trental (pentoxifylline) too. Take it thrice daily, 500mg tablet. There is no interaction with Omega 3 capsules.

But it also scary about Schamberg's purpura disease, when i gone through internet and came to know, which it will spread all over the body. Is there any strongest topical steroid cream which would give immediate results.

Coming to the trental tablet, which i had already taken 3 months (morning and evening) again you want me to take thrice daily and how long duration should i take it ?

And omega 3 should be taken morning or night.
Is there any proper way to diagnose and confirm it is Schamberg's purpura disease.

Also i will wear shoes and socks for more than 10 hours, will i need to apply this (betamethasone valerate 0.1%) cream during the period.

Hi.

Schamberg's purpura is commonly restricted to lower legs only. It may gradually increase in its extent but still is usually confined to lower legs. Sometimes it may also regress spontaneously, partially or completely. The condition is easy to diagnose clinically though a skin biopsy can confirm the diagnosis but it is rarely indicated because the diagnosis is clinically obvious.
You may apply a moderately potent steroid cream i.e either betnovate cream or momate XL cream, twice daily. No need for a more potent steroid cream. You may apply the cream twice daily, morning and night.
Yes, you may also take tablet Trental for 3-4 months. The dosage is 500mg, thrice daily.
