What Does Blood In Urine Indicate?

Question : Dr. XXXXXXX .. I was the one that was talking to you about that penis rash. It is now under control as I went to emergency and got some cream I guess it has a steroid solution mixed into it. They did a urine test and no infections however they did see a minute speck of blood but they told me it was so slight that it could have been their machine. Is this possible?? if it was blood I know it could be cancer but are there not other things that it could be.?? thank you XXXX

Brief Answer: Very possible if in minute quantities Detailed Answer: Hi and thank you so much for this query I am so sorry to hear about the rash you had. However, I am happy to hear that it is already well managed. Today, I will focus more on this urine test that showed some traces of blood and address the disturbing thoughts that this must have caused you. Blood in urine can suggest so many different conditions. It could just be an incidental finding that does not represent a disease process. this is even more likely when the amount is very minimal. Other possible causes are infections, kidney stones, clotting abnormalities, cancers(as you mentioned), kidney disease, bleeding disorders etc. In either of these conditions, the number and nature of the microscopic exam would be very suggestive as to what the possible cause of this condition is. With cancers, blood would be in thousands and not trace amounts. Because the number was small, disregarded by your doctors, but you are concerned about cancers, we would need a convincing result to put your mind at ease. I will suggest that we get another urine sample for analysis in a month's time. If anything worrisome should still come up, then we would need follow up. If nothing shows up then for sure it was most likely and incidental finding. I hope this addresses your query fully. thank you doctor you have put my mind at ease a bit. The word cancer scares everyone and my doctor is not a real people person if you know what I mean. As you mentioned that you would have to see thousands of traces of blood that would MAYBE suggest cancer. With that many traces would you not see it with the naked eye?? They told me that it was a very minute trace looking under a microscope and then not really sure if it was blood or their equipemnt. I did submit another sample yesterday and the doctor has scheduled me an altrasound. Just briefly getting back to my rash. My wife passed away in /98. I am not sexually active with women. I do however masturbate from time to time. Is it ok to use body lotion as a lube. thank you

I would like to thank you for your condolence on my wife. The pain never goes away, you just learn to deal with it. There was just one thing that I asked and maybe you missed. You had mentioned that if there were signs of cancer you would see thousands of traces of blood in the urine sample. If this was so, would I not see it when I pee. Or would it not show a redish color. As right now it looks normal to me. A bit yellow but then again I don't drink the amount of water that I should thank you

Good Morning Doctor... I am really sorry to bother you again. You seem to be a very intelligent doctor more so than I have seen in the past. I just want to ask you about this rash that I am dealing with on my Penis. It is going on now for about 6wks I have not been sexually active in the past 7yrs. I do masturbate from time to time but have never ran into this before. I am circumsized. It all started with 2 red dots on the head of my penis. I have an extremely large head witch is about 9" around. I thought that maybe I had broken a blood vessel. Then after reading on the internet a lot of men have this problem. The dots are gone but still irritated where the skin of the shaft meets the head of my penis. The only thing that I have changed in my diet is that I have been eating 2 handfulls of peanuts daily. Now I have stopped doing that for about the past 2 weeks. The doctor in emergency gave me some cream Fucidin H crm 2/1% to use for only 5 days. Only problem with this cream is that it seems to be thinning the skin. Now My Doctor says to keep using it. However I stopped after 5 days giving it time to maybe heal on its own. Do you have any suggestions as to what it might be?? Should I keep using the cream or leave it for a week to see how it looks. There is no hurting, no itching nothing it is just there. I would appreciate anything you can tell me. Again thank you

Brief Answer: Thanks for the piscture! Detailed Answer: Hi and thank you so much for following up with me. I have taken a look at the picture. Unfortunately, it is just so difficult to see the rash. I can however see the area of discoloration/depigmentation. This is no surprise as your doctor prescribed a steroid cream that basically does this. This is the same product that blacks used when then want their skin color to become lighter. I will strongly recommend that you stop this drug. At this time, I will suggest that you allow the rash without applying any medication to it. Always strive to keep it clean, dry and airy and let's see what it turns out to be. Your sexual history makes it very less likely for this to have been caused by an infection. IF you had reported itches, I could have leaned towards a fungal infection but that is not the case right now. I may tend to suspect some of the lube you had used for masturbation for causing some form of allergic reaction here. Let's give ourselves sometime of not manipulating the rash, no application of any cream, keeping it cleaned and dry and no masturbation for about two weeks to see exactly what we come forth with. This would provide more information as to what the possible cause of this is. I am aware that you are only allowed to talk up to the 12th of this month. Would like you to update me shortly before your time expires and if need be you may want to keep the communication line open for more contribution. I wish you well. Looking forward to the updates. The doctor should have told you what the initial findings are and what he plans on doing next for your consent. He cannot take the decision to carry on just any test on a urine sample from a patient without his consent simply because he has the sample at his disposal. Did he call you to inform you well ahead of time that your test had some abnormal findings that may warrant further investigations? The right thing to have done is to discuss the test findings with you and then suggest what he thinks should be done. Did he at least produce the results of the test that prompted further investigation? He should!!! Am confused myself. So what did he then say you should do or when would the results be back? I don't want to question his professionalism and ethics on this. I am not very happy with whatever he is doing without your consent and without informing you every step along the way. I agree with you totally. He is scaring the hell out of me with all this cancer talk. I honestly don't believe he remembers me leaving a sample there. No one called me I called in to the office and made an apt. with him. See when I left him he gave me the form for the ultra sound. It was his receptionist that told me to leave a sample and I think that is where he is getting mixed up. He has only been my doctor for the past 4 months as he has taken over from the other retired doctor. I am not hiding anything I am a smoker and I know that is bad. But it is a bad habit to quit. But he just seems to getting on my nerves that everything resolves around smoking cancer this and cancer that.. He said if anything comes negative he will call me in. We will see because I really don't think he did anything with the sample i left as he knew I was going for an altra sound anyway. Getting back to that rash ... on my penis I have stopped using that Fucidin cream for the past 3 days now. Don't take me the wrong way but I woke up with a very hard erection and sometimes that does happen and i noticed the skid on the head was all scaly more like dead skin kind of flaky. I don't know if this is a sign of healing. Maybe you could shed some light on that. No pain no itching nothing. I do eat eggs every morning and I have also been eating red grapes for the past 3 months everyday. again thank you for listening to me as you do put me at ease...

Your diet has nothing to do with this. Keep eating healthy and that's what matters. I understand you smoke. It is your right to and I will not condemn you for that. However, quiting is of so many benefits to you. I will recommend you think over it and if quiting is something you would like to try, let me know. Brief Answer: I am happy to hear you want to try to quit Detailed Answer: Hi and thanks for expressing this wish of yours to try and quit. This is the single and most difficult step to take. In itself, it is a reason to celebrate and congratulate yourself on making progress. I will not ask you to quit overnight. My first step would be for you to cut down your daily consumption. If you have been taking 5/10/15 cigarettes daily, I will suggest that you start be dropping the number. Keep dropping it until you get to zero at your convenience within say 1-2months. It is a very non conventional way but it is my style. I love it simple and doable. You know what the disadvantages of smoking are. You know what the advantages of not smoking are. What are some of the persons that matter most to you? Some of the fun things that you love? Imagine being unable to accomplish them because of some complications related to smoking? Have you ever thought of not being there for your son/daughter/grand children, loved ones because of an early exit from this world related to smoking? I know what you mean by habit. I could never relate to it until I got hooked up to video games while in the fourth year of medical studies. Only then did I understand what it means when somebody say I find myself there even against my wish. I tried stopping several times without success before finally making my way out of it. The determination is what matters and how many times you fail doesn't matter but all that matters is the final result......Being able to stay clear of smoking. I know it is not an easy journey. I know there are a lot of challenges that you would face down the road. There would be side effects and they would not be very friendly. However, when you think of what lies ahead, huge benefits that cannot be obtained otherwise......you would never fail because you will never quit trying until you quit smoking. I am optimistic that you would walk this journey, not because it is easy but because you would never walk alone. You can count on me to be there as often as you need me. I am not angry because you smoke but would be happier knowing you no longer smoke. I wish you well. I think I can continue writing forever but you would get more information as we continue the journey. thank you for your statement. It really does make sense and I think my first mistake is the fact I never really kept track of how much I have been smoking so I believe that is a good starting point as that may really open my eyes. So that is where I am going to start. Keeping track of my smoking may just work. Just to touch again on this rash. I spoke to a guy that had a similar rash. He was not circumsized. The doctor told him that he needs to have it done. He did that but the rash never went away. He used coconut oil for about a week and has never had trouble again. Have you ever heard of this. Just don't understand what this rash could be caused from. Like you said it is not my diet. I have changed nothing other than eating peanuts and I do eat red grapes daily. I just wish I could figure it out. I am under a lot of stress right now I don't know maybe it is that. again thank you for your help thanks again

If you look right where the skin joins to the head of the penis that is where the skin looks thin. Also have a red patch on the tip of the head but hard to see. I have been reading a lot about this balanitis. Is this possible even though I am circumsized. Having a bit of trouble with this camera so I hope you get these. thankyou I have sent new pictures but even after I reviewed them it is still hard to see. You are a professional and I know what the problem is. I told my doctor the same thing you can see the rash much better when I have an erection. He told to try and he left the room but it did not work. Not too easy to get an erection in the doctors office. I really need some direction so if you would not be offended I would like to send some images when it is erect. will view them and if it does not show it better I will not send them. I only have another 3 days to talk to you. I will not send them unless you give me the ok thank you

I have sent you 3 or 4 pictures.. hopefully you get them all. There is one where you can see the redness in the center of the head of the penis. Then the other ones if you look right where the skin is attached to the head it is a darker color almost as if the skin has thinned out. Maybe that is from the different creams. The redness on the one picture on the head is rough and scaley. I have never had anything like this before. It does not seem to bother me at all it is just there and it bothers me. Is it possible that I have some kind of illness that I don't know about. Like I said all my blood work came back good. My 2nd Urine test shows negetive. Should my doctor be sending me to a dermatologist. ?? thank you for caring

For the black spot, it is a side effects from the steroid creams. For the red rash on the head of the penis, this is not too typical of any disease condition. My honest advice remains that you stop all these products and see what this rash turns into. If you should get any worsening or no amelioration, seeing a dermatologist for direct examination and investigation would be a great idea. I understand you are much bothered by the presence of this rash. however, tolerating it for now and now and more so because it is not causing any symptoms would be vital in understanding what is going on and providing solutions. My question is the altra sound is to look at the blatter. I receive every message and just want the right time to respond with the right information. i have seen so many rash type. some very typical and common with easy treatment options and some that do not fit any typical rash. The most important approach to non typical rash is to avoid rushing to treatment without understanding what is going on. Most of them get resolved without any aggressive treatment and they are never an emergency. I know I have not resolved this problem already but would see what we do before this month expires. sorry I have so many messages I figured that since I re paid my dues I would have to go with another doctor which is something I did not want to do. Yes by all means you can address me as XXXXXXX I live in a small town called Kingsville Ontario Canada. I am just south of Detroit Michigan. Just wanted to let you know that I think the skid where it is attached to the head of my penis is looking better, not so dark in color. So I hope that is a sign of healing. The rash on the head is still there. From what you are saying this rash may go away on its own and never come back but then again it could come back. thank you for getting back to me and I will stay in touch.

thank you for your answer. That was the one thing I was trying to avoid was talking to another doctor. I would have to through the entire story again and that would get me no where. You have been a great help to me and that is why I paid the 15 dollars to continue to talk to you and only you. However, I am happy to hear that it is already well managed. Today, I will focus more on this urine test that showed some traces of blood and address the disturbing thoughts that this must have caused you. Blood in urine can suggest so many different conditions. It could just be an incidental finding that does not represent a disease process. this is even more likely when the amount is very minimal. Other possible causes are infections, kidney stones, clotting abnormalities, cancers(as you mentioned), kidney disease, bleeding disorders etc. In either of these conditions, the number and nature of the microscopic exam would be very suggestive as to what the possible cause of this condition is. With cancers, blood would be in thousands and not trace amounts. Because the number was small, disregarded by your doctors, but you are concerned about cancers, we would need a convincing result to put your mind at ease. I will suggest that we get another urine sample for analysis in a month's time. If anything worrisome should still come up, then we would need follow up. If nothing shows up then for sure it was most likely and incidental finding. I hope this addresses your query fully.
