Brief Answer:
You need further evaluation.

Detailed Answer:
Hello XXXXX
Thanks for writing to HCM

According to report, there is a well circumscribed lesion in the upper pole of ovary which is heterogeneous in echotexture.Lesion may be due to many reasons.
Size, side and nature(solid, cystic or mixed type) of lesion is not mentioned in the report.
You need further evaluation like tumour markers,Contrast CT scan of abdomen.Treatment depend upon clinical findings and investigation reports.
You don't worry at all.Lesion may be benign in nature.Exact nature can only be confirmed by further evaluation.

Endometrium is also thickened. You need FNAC/biopsy of endometrium. It is important due to tamoxifen intake, your age and history of post menopausal bleeding.Tamoxifen has been linked to endometrial cancer in some women so thickened endometrium requires further evaluation.

Hope I have answered your question.
Further queries are most welcome.
