Thank you for asking on HCM!

If he is coughing up fluid this may be related to pulmonary edema.

I recommend bringing him to the ER for a careful physical exam and some tests:

- a chest X ray study
- an ECG
- a cardiac ultrasound
- an arterial blood gas analysis
- some blood lab tests (blood electrolytes, complete blood count, kidney and liver function tests).

He may need to be treated in the ER service with intravenous drugs.

If this is a chronic situation, an optimization of heart failure therapy is probably needed.

You should discuss with his doctor on the above issues.
