Question : i am 45 years old and my period is about a week late. Could it be menopause or pregnancy. I took a pregnancy test and it came up negative

It is most likely due to perimenopause but if you have a doubt than wait for a week and get your repeat urine pregnancy test.
You can go for blood beta hcg test to confirm the result ( more confirmatory test).
